David - per our conversation last week - attached is a photo of a similar crossing the contractor did somewhere else.
Andrew - I realized that I did not copy you on this last week, so please see below. The contractor is planning on installing
some temporary sheet pilings around either side of the live gasline to protect it during the open cut of the Green River.
There will be no change what -so -ever in the permitted impact footprint, but since this stuff wasn't depicted on the
original plan, so we are having them generate a drawing for your record. Their rough notes are attached for additional
reference as well.
I'll forward the clean drawing to you as soon as we get it, and please don't hesitate to call me if you have any questions -

Hey David thanks for speaking with me today. Per our conversation, the contractor had some concern about exposing
the existing live line at the Green River crossing, so are taking some additional precautions. This is a sketch that they
provided. The sheet pilings will protect the existing pipe, and the bridge will let them string the new pipe and excavate.
All will be conducted within the existing permitted footprint - no additional temp workspace is required - no new
impacts, just changing the method of installation slightly.
I will be able to get you a better drawing for the record in the next few days. Thanks again for your help -j

Line -Pipeline Replacement Project
Polk, Henderson and Buncombe Counties, North Carolina

Dear Mr. Brown:
This letter is in reference to the Line T-01 Pipeline Replacement Project located in Polk, Henderson and
Buncombe Counties. In accordance with a special condition of project authorization under Nationwide
Permit No. 12, issued by your office on November 19, 2015, this letter represents the first of what will
hereafter consist of bi-weekly (once every two weeks) reports summarizing construction progress of the
project.
The primary construction contractor is Latex Construction Company (Latex). Although mobilization and
equipment staging operations commenced around the first week of January, project construction did not
officially commence until January 25, 2016. S&ME currently has three Environmental Inspectors assigned
to the project.
Relevant pre -construction activities (January 11— 24) and a summary of work that has occurred since the
official start of construction (January 25- February 6) are listed below:
+ Project has been broken into three phases for purposes of NPDES reporting: Phase 1 (Stations 0-390)
with rain gage at Green River Cove Road; Phase 2 (Stations 390-830) with rain gage at Chestnut
Stump Road; Phase 3 (Stations 830-1336) with rain gage at Lewis Road.
+ S&ME Environmental Inspectors marked streams and wetlands with signage and photographed
existing conditions in addition to marking Access Roads with signage and photographing existing
conditions.
+ An environmental orientation and pre -construction meeting were held on January 19, 2015 at PSNC
Hendersonville office. Orientation presented by S&ME personnel to Latex personnel (foremen -level
and above), S&ME Environmental Inspectors, PSNC personnel, and additional PSNC third -party
Inspectors. Mandatory NCDEQ Pre -construction meeting was scheduled for that afternoon at the
same location; however, NCDEQ personnel did not attend.
Bi -Weekly Status Report: January 11- February 6, 2015 Line T-01 Replacement Project
+ Latex plans to install some segmentable temporary sheet piling in Green River adjacent to existing
pipe to prevent undermining during installation of new pipe. S&ME performed hydraulic study and
determined that flow would not be altered substantially if the piling is installed as proposed and the
upstream dam releases were ceased during construction. Latex estimates that pipe installation can be
accomplished within 3-5 days if this approach is used. PSNC to notify Duke Energy regarding lake
release scheduling.
+ S&ME met with Cathy Ruth (Polk County Planning and Zoning) on January 20, 2016 to view pre -
construction conditions at the Green River and Ostin Creek crossings and discuss floodplain
encroachment mitigation measures.
Clearing — Latex Cleared ROW for Cane Creek HDD (Sta 1169+90 —1218+00) and mobilized to
Bright's Creek Golf Course (Sta 388+00). Began clearing ROW at Bright's Creek Golf Course, across
the Green River, and towards Silver Creek Road (Sta 388+00 —187+71) installing temporary bridges at
streams and timber mats across wetlands.
Environmental crew — Installed ECDs for Cane Creek HDD (Sta 1172+00 —1191+00) per plans.
Installed ECDs for entry pad at Wetland WC -19 HDD/Clear Creek HDD. Began installing ECDs at
Bright's Creek Golf Course towards the Green River (Sta 388+00 — 276+00) per plans.
+ Grading/ROW — Placed timber mats along ROW for access to Cane Creek HDD (Sta 1169+90 -
1190+00) including entry pad, exit pad, and temporary bridges over Streams SD -35 and SD -36 and
Wetlands WD -32 and WD -33. Placed timber mats for entry pad and along ROW for access to
Wetland WC -19 HDD (832+50 — 846+29) and Clear Creek HDD (Sta 847+02 — 855+06). Began
grading ROW at Bright's Creek Golf Course (Sta 388+00 — 308+00).
+ Road bore crew mobilized to Clubhouse Lane at Bright's Creek Golf Course on February 6, 2016 and
began preparing for road bore under Clubhouse Lane.
+ Major rain event (4.0" on February 3, 2016); numerous ECDs overwhelmed on Bright's Creek Golf
Course, with some sedimentation onto the golf course. Contractor began repairing ECDs and working
on removing off-site sedimentation on February 4, 2016, and is currently working on a plan with the
golf course operations director to clean up sediment on the driving range.
+ Daily reports provided by S&ME Environmental Inspectors indicate that construction operations to
date have been conducted in accordance with applicable conditions of the aforementioned permits,
with no deficiencies identified except as noted in the bullet above.
The next bi-weekly report is scheduled for submission on, or about, the week of February 22, 2016. If you
