Everett, Ph.D. Director Subject: Permit No. NCO077615 Authorization to Construct Windmill Fuel Stop Wastewater Treatment Facility iredell County Dear Mr. Davis: A letter of request for an Authorization to Construct was received February 14, 1990 by the Division and final plans and specifications for the subject project have been reviewed and found to be satisfactory. Authorization is hereby granted for the construction of a 25,000 GPD extended aeration type wastewater treatment plant consisting a 4,000 gallon grease trap, a 10,000 gallon pump tank with dual 100 gpm grinder pumps, a 15,000 gallon flow equalization tank with dual 115 gpm pumps and a 30 cfm blower, a 24,350 gallon aeration basin with two (2) 198 CFM (5 hp) blowers, a 6,553 gallon clarifier with sludge return air lift, a 5,600 gallon aerated sludge holding tank, a 1,520 gallon chlorine contact basin with a tablet type chlorinator, and all the associated piping, valves, appurtenances with discharge of treated wastewater into Third Creek • classified C waters. This Authorization to Construct is issued in accordance with Part III paragraph B of 1\PDES Permit No. NCO077615 issued January 2, 1990, and shall be subject to revocation unless the wastewater treatment facilities are constructed in accordance with the conditions and limitations specified in Permit No. NC0077615. The sludge generated from these treatment facilities must be disposed of in accordance with G.S. 143-215.1 and in a manner approved by the North Carolina Division of Em-iron men tal Management. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take immediate corrective action, including those as may be required by this Division, such as the construction of additional or replacement wastewater treatment or disposal facilities. Such notification to the regional supervisor shall be made during the normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays. Box 27687, Raleigh, North Carolina 27611-7687 Telephone 919-733-7015 An Equal Opportunity Affirmative Action Employer Mr. Davis May 15, 1990 Page Two Upon completion of construction and prior to operation of this permitted facility, a certification must be received from a professional engineer certifying that the permitted facility has been installed in accordance with the NPDES Permit, this Authorization to Construct and the approved plans and specifications. Mail the Certification to the Permits and Engineering Unit, P.O. Box 27687, Raleigh, NC 27611. The Permittee shall employ a certified wastewater treatment plant operator to be in responsible charge of the wastewater treatment facilities. The operator must hold a certificate of the grade at least equivalent to the classification assigned to the wastewater treatment facilities by the Certification Commission. Within thirty days after the wastewater treatment facilities are 50% complete, the Permittee must submit a letter to the Certification Commission which designates the operator in responsible charge. A copy of the approved plans and specifications shall be maintained on file by the Permittee for the life of the facility. The Permittee shall provide adequate protection at the outfall to prevent erosion. The engineer's certification will serve as proof of compliance with this condition. A leakage test shall be performed on the pump tank and grease trap to insure that any exfiltration occurs at a rate which does not exceed twenty (20) gallons per twenty-four (24) hour per 1,000 gallons of tank capacity. The engineer's certification will serve as proof of compliance with this condition. The Permittee shall submit a sludge management plan for approval within sixty (60) days of permit issuance. Failure to abide by the requirements contained in this Authorization to Construct may subject the Pemlittee to an enforcement action by the Division of Environmental Management in accordance with North Carolina General Statute 143-215.6. y The issuance of this Authorization to Construct does not preclude the Permittee from complying with any and all statutes, rules, regulations, or ordinances which may be imposed by other government agencies (local, state, and federal) which have jurisdiction.
