General Project Narrative: The proposed project involves the replacement of Sampson County Bridge No. 810013 over Crane Creek on SR 1147 (Mt. Gilead Church Rd.) The existing bridge consists of 2
(Description of Minimization of Water @ 18' - 5" reinforced concrete deck spans on steel 'I' Beams. The proposed bridge is a 1@ 45' - 0", 1 Qa 35' - 0", 21" Cored Slab Bridge with a total width of 33' at the same
Quality Impacts) location. The project also includes 0.10 miles of roadway improvements. There are no existing stormwater facilities on this project other than grass swales on the west side of
the stream. The proposed project utilizes a'V' ditch in the same quadrents that outlets into the wetlands via variable width base ditches at 0% grade to reduce velocities. Also
since deck drains are not proposed on this project all storm water from the bridge and approach slabs will be collected by a storm drainage system. The system will discharge at
the base of the fill slope into a Class 'B' riprap pad @-L- Sta. 16+25.5 (LT).

Attached is the LIB permit application package for Sampson Bridge 13. This application is for NWP 3/WQC 3883.
The dewatering plan that was requested for this permit application is shown on the erosion control plans in this
package. Also, Stoney and the DOT Lead Engineer have not signed Attachment G and have asked that it be labeled
DRAFT because NCDOT is still completing their internal Cultural Resources Review. If you need signatures in order to
provide approval for this project, we will provide a signed Attachment G once their cultural review is complete.
