Comment: The permit expired July 31, 2020. A request for rescission was submitted by
the Town
of Dallas on 4/13/2022. The Town is requesting the permit be rescinded and replaced
with a Certificate of Coverage under general permit NCG59000000.

Comment: The Treament lagoon is overtopped capacity. There is water overtopping the berm
adjacent tot he walkway across from the drying beds.
At the time of the site visit, the drying beds had standing water and sludge in place. By
visual observation, it appears the drying beds are not draining fluid and may require
maintenance/reconditioning in order to drain properly.
There is a storm drain adjacent to the lagoon. With the lagoon currently at holding
capacity, there is a significant chance for fluid from the lagoon to travel to the storm
drain creating an unpermitted discharge.
The conditions were also indentified at this facility during the 2022 inspection.
