Dear Mr. VanStaalduinen:
In accordance with your application received May 7, 2024, we are forwarding
herewith Well Construction Permit No. WS0701198, dated May 23, 2024, issued to Carl
VanStaalduinen for the construction of one (1) water supply well on property owned by
VanStaalduinen Brothers LLC at approximately 0.5 miles south of 3810 Terra Ceia Road,
Pantego, Beaufort County, North Carolina (PIN: 6667-67-8319) at the approximate
latitude/longitude of 35.59274,-76.75907.
This Permit will be effective from the date of its issuance until May 23, 2025, and
shall be subject to the conditions and limitations as specified therein. If any parts,
requirements, or limitations contained in this Permit are unacceptable to you, you have
the right to an adjudicatory hearing before a hearing officer upon written demand to the
Director within 30 days following receipt of this Permit, identifying the specified issues
to be contended. Unless such demand is made, this Permit shall be final and binding.
Groundwater information collected in the coastal plain counties indicates
declining groundwater levels in the Cretaceous Aquifer System. In an effort to protect
and conserve the groundwater resources, the Division encourages water users to develop
and implement a water conservation and management plan.

A Well Construction Record (GW-1) must be filled out by the driller and
submitted to Water Resources, Attn: Information Management, 1617 Mail Service
Center, Raleigh, NC 27699-1636 and Water Resources, Attn: CCPCUA Program, 1611
Mail Service Center, Raleigh, NC27699-1611, within 30 days upon completion of the
well construction.

PERMIT FOR THE CONSTRUCTION OF A WATER SUPPLY WELL OR WELL SYSTEM
(Wells or Well Systems with a Design Capacity of 100,000 Gallons Per Day or Greater)
In accordance with the provisions of Article 7, Chapter 87, North Carolina General Statutes,
and other applicable Laws, Rules and Regulations:
PERMISSION IS HEREBY GRANTED TO

FOR THE CONSTRUCTION OF one (1) Water Supply Well in the Tertiary
Aquifer System in Beaufort County, North Carolina. The well will be located on property
owned by VanStaalduinen Brothers LLC at approximately 0.5 miles south of 3810 Terra
Ceia Road, Pantego, Beaufort County, North Carolina (PIN: 6667-67-8319) at the
approximate latitude/longitude of 35.59274,-76.75907. This Permit is issued in
accordance with the application received on May 7, 2024, in conformity with
specifications and supporting data, all of which are filed with the Department of
Environmental Quality and are considered integral parts of this Permit.
This Permit is for well construction only, and does not waive any provision or requirement
or any other applicable law or regulation.
Construction of any well under this Permit shall be in strict compliance with the North
Carolina Well Construction Regulations and Standards (15A NCAC 02C .0100), and other
State and Local Laws and regulations pertaining to water supply well construction.
If any requirements or limitations specified in this Permit are unacceptable, you have a
right to an adjudicatory hearing upon written request within 30 days of receipt of this Permit.
The request must be in the form of a written petition conforming to Chapter 150B of the North
Carolina General Statutes and filed with the Office of Administrative Hearings, 6714 Mail
Service Center, Raleigh, North Carolina 27699-6714. Unless such a demand is made, this
Permit is final and binding.
This Permit will be effective for one year from the date of its issuance and shall be subject
to other specified conditions, limitations, or exceptions as follows:

1. The Permitee shall notify Ryan Wolbert of the Washington Regional Office by email
at ryan.wolbert@deq.nc.gov or by phone at (252) 948-3849, at least three business days
in advance of the start of well construction.
2. The Permitee shall notify Ryan Wolbert of the Washington Regional Office by email
at ryan.wolbert@deq.nc.gov or by phone at (252) 948-3849, at least 24 hours in
advance of grouting of the well.
3. The well(s) shall have a Well Contractor Identification Plate and a Pump Installation
Information Plate in accordance with 15A NCAC 02C .01070)(2) and (3).
4. Well construction records (GW-1) for each well shall be submitted to the Division of
Water Resources' Information Processing Unit and CCPCUA Program within 30 days
of the well completion.
5. When the well(s) are discontinued or abandoned, they shall be abandoned in
accordance with 15A NCAC 02C .0113 and a well abandonment record (GW-30) shall
be submitted to the Division of Water Resources' Information Processing Unit within
30 days of the well abandonment.
