Re: I-77 Welcome Center WWTP
Surry County
Dear Ms. Williams:
Please find enclosed a completed Wastewater Pump and Haul Form P&H 11-23 and check for the $1,160
application fee.
The I-77 Welcome Center in Surry County is served by an existing permitted 30,000 GPD WWTP (NC0029190).
The existing WWTP has failed and has been taken off-line for repairs. The Welcome Center facility has been closed
until alternate means of wastewater disposal is available. Accordingly, NCDOT would like to implement temporary
pump and haul measures so that the Welcome Center facility can be reopened.
NCDOT has received confirmation (attached) that the City of Mount Airy WWTP is willing to receive and treat
wastewater hauled from the Welcome Center. The City has also provided NCDOT with the list of approved septic
haulers who are currently authorized to discharge at the Mount Airy WWTP:
P.D Quik - Donald Goins — 336-401-0459
V & S Septic Service — Benny Stamper — 336-786-2402
JW Septic Service — Jeremy Williamson — 336-325-0899
Donald Beck Jr Septic Service — Donald Beck Jr. — 336-353-8060
Bare's Backhoe & Septic — Matthew Bare — 336-401-7195
Septic 911 —Reid Fulk — 336-985-7051
Foothill's Septic Service — Kamron Naylor — 336-618-5904
NCDOT has contacted the first firm on the list, P.D. Quick, and confirmed that they have the capability of providing
the required pump and haul services (copy of email attached). NCDOT would also like to have the ability to call
upon any of the other approved haulers to provide assistance if needed.
NCDOT's I-77 Welcome Center WWTP includes two (2) parallel 15,000 GPD package treatment plants. One of
these units will be used to temporarily store the wastewater for the pump/haul operations while the other treatment
unit can be repaired and placed back in service.
Water billing records for the Welcome Center indicate that the facility's average daily flow has been about 5,100
gallons/day. The approximate holding volume of one package plant unit is estimated to be about 22,000 gallons
(aeration basin plus clarifier compartment plus sludge holding compartment). When the Welcome Center is open,
facility maintenance staff members are on -site and available to monitor liquid level in the tank. The required
duration for the pump and haul operations is unknown and will be dependent upon how long it takes to complete the
WWTP repairs, but it is estimated to be at least one month. It is understood that pump and haul permits are not
issued for durations longer than 6 months. NCDOT would like to have the ability to utilize the pump and haul
solution any time it becomes necessary within the next 6 months. NCDOT intends to get the WWTP back in
operation as soon as possible, but unknowns related to availability or delivery time of key components may impact
that timeline and there's always the possibility of a future breakdown after it goes back into operation.

Please let us know if you have any questions or require anything additional in order to process NCDOT's request to
initiate pump and haul operations. If at all possible, NCDOT would like to have the requested authorization in place
early in the week of May 20`h so that the Welcome Center facility can be re -opened in advance of the Memorial Day
holiday weekend.
Sincerely,
THE LANE GROUP

Re: 1-77 Welcome Center
Dear Mr. Stankiewicz:
In followup to our recent telephone discussions, PD Quick Septic Hauling is agreeable to provide septic tank
pumping and hauling services to NCDOT at the 1-77 Welcome Center WWTP on an as -needed basis. It is our
understanding that the Welcome Center's WWTP is temporarily out of service and will remain off-line until
necessary repairs can be made. The existing WWTP treatment unit tanks will be used as temporary storage
basins to hold wastewater from the Welcome Center. We will pump the wastewater from these tanks and
transport it to the City of Mount Airy's WWTP for treatment.
PD Quick Septic Hauling is an approved septic hauler permitted to discharge at the City's WWTP. We
operate using 4 pumper trucks from 2000 to 4000 Gallons in size. We understand that the wastewater flows
generated at the Welcome Center will fluctuate on a day-to-day basis, but historical water billing records
indicate that the flows typically average 4,000 - 7,000 gallons per day. Wastewater deliveries to the City's
WWTP will be coordinated with the WWTP operators and will be completed in accordance with the City's
standard requirements for receipt of septage.
Let me know if you need anything additional and we look forward to assisting the NCDOT.
