Existing Sewer Pump Station:
The existing Dutchman Creek Sewer Pump Station was originally permitted under WQ0004726 approved
on March 28, 1991. The current setup consists of a duplex pump station containing two (2) 34 HP — 210
gpm pumps, 10' diameter wet well, valve vault, 6-inch piping and appurtenances, a revolving crane,
standby diesel generator, electrical wiring/panels, SCADA, and controls. The existing pump station
discharges to an existing 6-inch force main (approximately 5,450 LF traveling south) which empties into
an existing 8-inch gravity sewer along Highway 158.
Proposed Sewer Pump Station Modifications:
Proposed modifications to the existing Dutchman Creek Sewer Pump Station include the following:
• Replace existing pumps with two (2) new 38 HP — 450 gpm pumps to allow for future
development upstream of the pump station
• Remove/replace existing wet well and valve vault hatches with watertight access hatches due to
location in the 100-year floodplain
• Remove/replace existing 6-inch station piping, valves, and appurtenances
• Install new electromagnetic flow meter in a new precast concrete meter vault
• Remove/replace existing standby diesel generator sized to power new pumps
• Remove/replace existing electrical wiring, control panels, and SCADA with new
• Remove existing revolving crane

gravity sewers, approximately 6,391 linear feet of 15-inch gravity sewers and approximately 1,658
linear feet of 16-inch gravity sewers to serve Davie County Wastewater Improvements and the
discharge of 25,000 GPD of collected domestic wastewater into the Davie County Board of
Commissioners' existing sewerage system, pursuant to the application received February 18,
1991, and in conformity with the project plan, specifications, and other supporting data
subsequently filed and approved by the Department of Environment, Health and Natural Resources
and considered a part of this permit.
This permit shall be effective from the date of issuance until rescinded and shall be subject

ownership, or there is a name change of the Permittee, a formal permit request must be
submitted to the Division of Environmental Management accompanied by an application
fee, documentation from the parties involved, and other supporting materials as may be
appropriate. The approval of this request will be considered on its merits and may or may
not be approved.
• 6. Construction of the sewers, pump station(s) and force main shall be scheduled so as not to
interrupt service by the existing utilities nor result in an overflow or bypass discharge of
wastewater to the surface waters of the State.
Wo
• 7. Upon completion of construction and prior to operation of this permitted facility, a
certification must be received from a professional engineer certifying that the permitted
facility has been installed in accordance with this permit, the approved plans and
specifications. Mail the certification to the Permits and Engineering Unit, P.O. Box 27687,

9. Failure to abide by the conditions and limitations contained in this permit may subject the
Permittee to an enforcement action by the Division of Environmental Management in
accordance with North Carolina General Statute 143-215.6.
10. The issuance of this permit does not preclude the Permittee from complying with any and
all statutes, rules, regulations, or ordinances which may be imposed by other government
agencies (local, state, and federal) which have jurisdiction.
11. The Permittee shall provide for the pump station and force main the following items:

g. Flood protection if the pump station is located below the 100-year flood elevation.
12. In the event that the facilities fail to perform satisfactorily, including the creation of
nuisance conditions, the Permittee shall take immediate corrective action, including those as
may be required by this Division, such as the construction of additional or replacement
wastewater collection facilities.
13. Noncompliance Notification:
The Permittee shall report by telephone to the Winston-Salem Regional Office, telephone

The 50,000 gpd is now obligated as part of this request; it is part of the 0.847 MGD listed in I.g.
in the Flow Tracking for Sewer Extension Applications form. This just gives Davie County
permission from Mocksville to send this flow once it is permitted. The actual new flows will be
permitted by the individual developers once their plans are in place. This permit application is
just for the pump station upgrades, not for new flow. Please call me if we need to discuss.

Is the 50,000 gpd part of the obligated flow not yet tributary the WWTP? Is there a sewer
permit for the 50,000 gpd flow from the County to the Town? Thanks.

Cheng, these two sections are asking for two different numbers. The FTSE, section 1.f is
discussing allocated flow to the receiving treatment facility, which is 50,000 gpd in this
case associated with the developer building in the Dutchman Creek Pump Station
sewershed. This is the additional flow the County is requesting from the town of
Mocksville to discharge to their collection system.
Section VI of the FTA is asking how much flow is "generated" by this project. Pump
Stations don't "generate" flow in the way a developer/neighborhood, commercial
property or industry would, so in this case, that flow is reported at 0.

Kyle,
Thanks for the information.
Sorry I have another question about the flow tracking form: Section I. f. Total flow for this
specific request 0.050 MGD, while in the application Section VI. 7. and 8. State that 0
GPD flow will be generated by the project, was the 0.050 MGD an typo?

Cheng,
This project will not impact the stream near McClamrock Road. The new force main will stay
inside the DOT right-of-way above an existing 60-inch RCP culvert. See image attached
showing the approximate location of the proposed force main. Erosion control measures will
be installed at the top edge of the slope to prevent any impacts to the stream.
Once the 8-inch force main is installed and operational, the 6-inch force main will be cut at
both ends of the alignment (at the pump station and at Main Church Road) and filled with
flowable fill.

Kyle,
Thanks for the information.
While continue to review the application, I noticed that the proposed 8-inch force main
appears to cross a blueline stream (on the topo map), please explain the reasons why
401/404 permits are not needed as specified in Section IX. 5. of the application.
Please elaborate how the existing 6-inch force main will be abandoned -in -place (to
ensure that no one ties onto this line without a permit, as well as how it will be valved off
from the system to prevent any introduction of wastewater).

Aaron and Kyle,
I was able to find the original permit issued in 1991 which covered the construction of
the pump station (attached), therefore, the application needs to be edited as a permit
modification, not a new permit. I am sorry for it taking so long for the application
package to reach me and understand the frustration caused by it. Please provide the
information I requested in the previous emails, I will continue to review the application,
and will draft the permit once the information is received. Please let me know if you have
questions.

Cheng,
I think it may be best to jump on a call with you and your supervisor so that we can minimize
the continued email traffic I've seen over the past week. Davie County would like to complete
this review ASAP and having to go back now for a third time to get signatures from them is very
frustrating, especially for what seems to be a non -beneficial edit to the project. Please provide
Kyle a time where we can jump on a call to discuss any additional needs from your office for
this project to be reviewed.
Thank you,
Aaron
