This permit is issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the U.S. Environmental Protection Agency dated May 9, 1994. Please note the following permit changes: 1) Based on effluent test results consistently below laboratory detection limits, the Division has removed the limits for all volatile organic compounds (see effluent table). Please note that the quarterly monitoring requirements for these parameters remain unchanged. 2) Previously -required parameters listed under Other Organics/Metals have also been re- evaluated. Of these, only copper remains on your effluent table as a "monitor -only" parameter. If continuous flow monitoring is not feasible, the permittee shall record the approximate time that discharge began and ended (or provide a calibrated, totalizing flow meter). Discharge shall not contain floating solids nor shall it contain foam visible in other than trace amounts. Permit NCO079227 SUPPLIMENT TO EFFLUENT. LIMITATIONS AND MONITORING REQUIREMENTS A. (2) CHRONIC TOXICITY PERMIT LIMIT (QUARTERLY) The effluent discharge shall at no time exhibit observable inhibition of reproduction or mortality significant to Ceriodaphnia dubia at an effluent concentration of 90%. The permit holder shall perform at a minimum, quarterly monitoring using test procedures outlined in the "North Carolina Ceriodaphnia Chronic Effluent Bioassay Procedure" (revised February 1998) or subsequent versions or "North Carolina Phase II Chronic Whole Effluent Toxicity Test Procedure" (revised February 1998) or subsequent versions. The tests will be performed during the months of February, May, August, and November. Effluent sampling for this testing shall be performed at the NPDES permitted final effluent discharge below all treatment processes. If the test procedure performed as the first test of any single quarter results in a failure or ChV below the permit limit, then multiple -concentration testing shall be performed at a minimum, in each of the two following months as described in "North Carolina Phase II Chronic Whole Effluent Toxicity Test Procedure" (Revised February 1998) or subsequent versions. The chronic value for multiple concentration tests will be determined using the geometric mean of the highest concentration having no detectable impairment of reproduction or survival and the lowest concentration that does have a detectable impairment of reproduction or survival. The definition of "detectable impairment," collection methods, exposure regimes, and further statistical methods are specified in the "North Carolina Phase II Chronic Whole Effluent Toxicity Test Procedure" (Revised February 1998) or subsequent versions. All toxicity test results, required as part of this permit condition, will be entered on the Effluent Discharge Monitoring Form (MR-1) for the months in which tests were performed, using the parameter code TGP3B for the pass/fail results and THP3B for the Chronic Value. Test data shall be complete, accurate, include all supporting chemical and physical measurements and all concentration/response data, and be certified by laboratory supervisor and ORC or approved designate. Total residual chlorine of the effluent toxicity sample must be measured and reported if chlorine is employed for disinfection of the waste stream. Permit NC0079227 Should there be no discharge of flow from the facility during a month in which toxicity monitoring is required, the permittee will complete the information located at the top of the aquatic toxicity (AT) test form indicating the facility name, permit number, pipe number, county, and th a month/year of the report with the notation of "No Flow" in the comment area of the form. The report shall be submitted to the Environmental Sciences Branch at the address cited above. Should the permittee fail to monitor during a month in which toxicity monitoring is required, monitoring will be required during the following month. Should any test data from this monitoring requirement or tests performed by the North Carolina Division of Water Quality indicate potential impacts to the receiving stream, this permit may be re -opened and modified to include alternate monitoring requirements or limits. NOTE: Failure to achieve test conditions as specified in the cited document, such as minimum control organism survival, minimum control organism reproduction, and appropriate environmental controls, shall constitute an invalid test and will require immediate follow-up testing to be completed no later than the last day of the month following the month of the initial monitoring. Although permitted for a maximum of 0.124 MGD, flow has averaged about 0.033 MGD. Major contaminants include chlorinated solvents and metals. Treatment is effectively containing the plume but is anticipated to continue for some years (personal communication from consultant Dennis Willis to Joe Corporon, 08Feb00). The NPDES permit was last renewed on March 5, 1995 and expired on January 31, 2000. The Division has received the appropriate documentation for permit renewal (July 20, 1999). In 1995 Schlage's consultant, Capsule Environmental Engineering, demonstrated that the treatment system could meet chemical -specific limits without carbon filtering and petitioned the Division to remove filters. Permission was granted [reviewed by NPDES Engineer, Susan Wilson]. System operators discovered a problem with total flow measurement (effecting data for some unknown period prior to May 1998) when a newly installed totalizing flow n w� �me�ter disagreed with the old (effluent meter indicated -0.66 MGD; influent meter -0.33 '"1` - The consultant verified measurements by comparing new influent/effluent meters; corrected the problem by switching out the old meter. Fact Sheet NPDES Renewal Pa"'e I PERMITTING APPROACH • Verification of Existing Conditions -- The Division has verified that Beech Branch is a Class C- NSW stream; summer and winter 7Q10= 0.0 cfs and 30Q2 = 0.0 cfs; and that Beech Branch is not listed as a 303 (d) stream (Joe Corporon, December 3, 1999). Review of Permit Compliance Issues -- DMRs were reviewed for 1998 and 1999. Although periods of no discharge were noted, no permit violations were found. Division contacted Randy Jones (RRO) for status and Staff Report. RRO conducted a site visit in February 2000; report pending. • Review of DMR Data - The Division reviewed the DMRs for Chronic Toxicity testing. Records indicate one failure (June 1998) following start-up after re -piping, but the system passed TOX testing the following month, and for the subsequent five (5) quarters (through November 1999). Considering other parameters, the 1998-99 data indicate that all effluent - limited volatile organic compounds (VOCs) were "not detected" at lab detection limits 50 to 100 times below permit limits. Similar to VOCs, those parameters monitored as Other Organics and Metals were reported "not detected" for two years with the exceptions of single hits or low levels (at or near the analytical detection limits) for Cu, Fe, Pb, and Mn (see attached Effluent Data Summary). Based on these data, the Division feels it is reasonable to alter this permit. lN c�+2IL�`15 KCtQ-� 6-,�Pr 7 ' PROPOSED PERMIT CHANGES 2k Based on a reasonable -potential analysis of Other Organics and Metals, only lead was retained in the permit with a quarterly monitoring frequency. Lead was retained because it was detected once at 0.008 mg/L, a level above the NC action level of 0.007 mg/L (15 NCAC 2B .0200). Furthermore, based on two years of effluent data, the Division concluded that there is no reasonable potential for this treatment system to discharge VOCs as toxicants. Therefore, VOC monitoring remains in the permit, but monitoring limits have been removed. Schlage Rocky Mount Sludge Practices Very little sludge is formed in the wastewater treatment facility at Rocky Mount, North Carolina. The plant uses a settling tank, air stripper and final filter to treat water from a ground water recovery system. This system is designed to remove the chlorinated solvents that have contaminated the site under the former Schlage Lock manufacturing facility. No chemical sludges are formed by this process and the only sludge is silt that is carried from the recovery wells in the water stream. The treatment process is shown on the attached block diagram. Some solids are captured in the Separation Tank that is primarily to separate any chlorinated free product. These solids are drained with the solvent phase into the product collection drum. This solvent is processed through Safety Kleen. Any additional solids are captured in the bag filters at the end of the treatment system. This material coats the bags requiring eventual replaced and disposal. About six bags per month are generated. These bags have been analyzed for hazardous characteristics and been found nonhazardous. They are air dried and taken by the Site Manager to the Nash County Sanitary Landfill Convenience Center #7 for disposal as an industrial waste. Since this NPDES permit covers the discharge of pretreated, recovered wastewater from a ground water recovery system and no industrial process discharges, the information requested for industrial facilities (production data and a PPA) are not appropriate and not included. This was confirmed with you by phone on July 14, 1999. In addition, the second item under Section 9 of Short Form C-GW (alternatives to surface water discharge) was not included. Mr. Charles H. Weaver, Jr. July 19, 1999 Page 2 Since the last renewal in 1995, the ground water treatment system has functioned well with regard to the discharge under the NPDES Permit. Quarterly analyses of the discharge generally show no detectable organics, very low metals, acceptable suspended solids and oil and grease. The facility passes chronic toxicity on a quarterly basis as well. During the expiring permit period, three major changes took place at the site. The first change was the shut down and eventual removal of the activated carbon canisters from the treatment system. This action was approved by DENR with the transmittal of the 1995 NPDES renewal on March 6, 1995. The carbon canisters were removed from service on April 7, 1995 and returned to the supplier (Calgon) on July 2, 1998. No adverse effects to the NPDES water discharge have been noted due to the removal of the carbon system. The second change was the sale of the idle Schlage- facility to Godley Realty and Construction in February 1996. The building will be leased by Godley to tenants for warehouse -like operations. Schlage maintains control of the wastewater treatment facility and NPDES outfall and has access to the system's wells and piping. The sales agreement also defined no -build sections -on the property in order to protect the underground remediation system. The property sale is not expected to have any effect on the NPDES discharge. A Site Manager, employed by Schlage, operates the treatment plant and acts as a caretaker for the system. The third event was the replacement of the steel, underground recovery well collection piping with a fiberglass piping system due to serious corrosion problems with the steel system. This occurred during 1997 and required that the pump and treat system be idled from February 7, 1997 to March 9, 1998. During this period no ground water was treated in the treatment plant. The only discharges under the NPDES were from the treatment of water that was pumped from the manholes in order to accomplish the pipe replacement. These events were reported to and approved by the Water Quality Division. The system has functioned well since its return to service on March 9, 1998. Mr. Charles H.
