Following up, per your instructions, I mailed an original of the completed Form 2A to Ms. Therefore earlier this week.
With regard to the permit, I would recommend issuing the permit to The Village -Stone Highway Mobile Home Park, and
sending the permit to my attention as the park's owner.

Also, do you want the permit issued to Lynne Weaver [owner] or The Village [owner]?
Similarly, is the facility name still Stone Highway Mobile Home Park, or has it changed to The Village?

completed with all of the available information we have. Please let me know if the scanned form is sufficient, or if I

permit is not the one I was referencing.
NPDES permit NC0060623 is for the wastewater [sewage] discharge from the facility's wastewater treatment
plant. NC0060623 expired almost two years ago and must be renewed.
Thank you for paying the annual fee of $1150. That is one step toward renewing your permit. Completing and
submitting Modified Form 2A is the second, and more critical step toward renewal.
Please complete and submit Modified Form 2A so that NC0060623 can be renewed. As I mentioned before, work
with your ORC (Mr. Young) to complete the application.

Dear Mr. Weaver,
Thank you for your email. I am the executor of my late father Sterling A. Weaver's estate, and the heir to his
estate. It is my understanding that the permit is up to date; please see the attached permit, which was issued in
January 2024. A permit fee in the amount of $430 was paid by Truist check no. 137 on December 10, 2023. In
addition, on January 31, 2024, an annual permit fee in the amount of $1,150 was paid by Truist check number
154. Finally, please find attached a copy of the signed application that was mailed to NC DEQ in December 2023,
which was signed by me as owner, before mailing it in, and was presumably received by NC DEQ prior to the issuance
of the permit in January 2024.
I am assuming that everything is in order, other than the change in name to reflect my ownership of Stone Highway
MHP, rather than my late father. After reviewing the attached and double checking the receipt of funds, please let
me know if you need anything further.

our database accordingly, and now show you as the owner of record for permit NC0060623.
Unfortunately, we cannot send you a new permit at this time. The Stone Highway MHP WWTP permit expired on
4 30 2022. No permit renewal application was received by the Division prior to expiration. Any discharge of
wastewater from this facility is ILLEGAL without a valid permit.
The renewal application was due no later than 11/1/2021. The deadline is derived from Federal and State
requirements to request renewal 180 days before expiration of the current permit; the requirements are listed in 33
USC 1251, 40 CFR 122.21 (b), and 15 NCAC 2H.0105 (e). Since no renewal application was received, the Division has
no active permit that can be transferred to you.
Please complete and submit a renewal application so that your permit can be renewed and you can receive the
necessary documents to continue operation of the facility. You should enlist the help of your facility's Operator
(ORC) to complete the form. The renewal application form (Modified EPA Form 2A) can be found here:
