Division personnel have reviewed and approved your application for renewal of the subject permit. Accordingly,
we are forwarding the attached NPDES discharge permit. This permit is issued pursuant to the requirements
of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and
the U.S. Environmental Protection Agency dated October 15, 2007 (or as subsequently amended).
Division of Water Quality response to June 6, 2009 comments to draft permit;
1. Your request to change the Monthly Average Flow to 25.0 MGD has been approved.
2. Your request to change the monitoring frequency for Iron to Quarterly has been denied at this time. A
review of the facility DMR sampling data indicates your facility discharge has a Reasonable Potential to
exceed the Water Quality Action Level Standard of 1.0 mg/L for Iron.
3. Your request for inclusion of additional language in footnote #6 —"Ammonia nitrogen monitoring only
applies if the facility chloraminates" is approved and included in the final permit.
4. Your request to remove Special Condition A. (3). — Facility restart of the renovated Thomas Plant has
been approved for the reasons noted in your June 6, 2009 letter.
5. Your request to change the Latitude and Longitude on your permit site map has been approved.
6. Any facility can request that the Division reduce or eliminate monitoring requirements contained in
NPDES permit. Facilities must submit at least 12 or more sampling results for effluent parameters to
be considered for reduction or elimination. The Division will review these sampling results and make a
determination concurrent with the request.

If any parts, measurement frequencies or sampling requirements contained in this _permit are unacceptable to
you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt
of this letter. This request must be in the form of a written petition, conforming to Chapter 150E of the North
Carolina General Statutes, and filed with the Office of Administrative Hearings (6714 Mail Service Center,
Raleigh, North Carolina 27699-6714). Unless such demand is made, this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may require

This permit and authorization to discharge shall expire at midnight on June 30,
2014.

SUPPLEMENT TO PERMIT COVER SHEET
All previous NPDES Permits issued to this facility, whether for operation or
discharge are hereby revoked. As of this permit issuance, any previously issued
permit bearing this number is no longer effective. Therefore, the exclusive authority
to operate and discharge from this facility arises under the permit conditions,
requirements, terms, and provisions included herein.
Winston-Salem / Forsyth County Utilities is hereby authorized to:
1. Continue to operate a conventional water treatment plant discharging filter -
backwash wastewater. This facility is at the R. A. Thomas WTP, located at
1201 South Martin Luther King, Jr. Drive near Winston-Salem, in Forsyth
County.
2 Discharge from said treatment works at the location specified on the
attached map into an Unnamed tributary to Salem Creek, classified as C
waters in the Yadkin -Pee Dee River Basin.

Footnotes:
1. Limit and monitor only if the facility adds chlorine to water that is eventually discharged.
2. The Division shall consider all effluent TRC values reported below 50 ug/l to be in compliance
with the permit. However, the Permittee shall continue to record and submit all values reported
by a North Carolina certified laboratory (including field certified), even if these values fall below 50
ug/l.
3. Fluoride monitoring applies if the facility backwashes with fluoridated, finished water.
4. Zinc should only be monitored if a Permittee uses zinc orthophosphate as a corrosion inhibitor.
5. Parameters should be monitored in conjunction with toxicity test.
6. Facilities using chloramination will be required to monitor ammonia nitrogen on a quarterly
basis. Ammonia Nitrogen monitoring only applies if the facility chloraminates.
7. See Attachment A (2.).
All samples collected should be from a representative discharge event.
There shall be no discharge of floating solids or visible foam in other than trace
amounts.

A. (2.) CHRONIC TOXICITY MONITORING (ORTRLY)
The permittee shall conduct quarterly chronic toxicity tests using test procedures outlined in the "North
Carolina Ceriodaphnia Chronic Effluent Bioassay Procedure," Revised February 1998, or subsequent
versions.
The effluent concentration defined as treatment two in the procedure document is 90%. The testing shall be
performed as a Ceriodaphnia dubia 7day pass/fail test. The tests will be performed during the months of
January, April, July and October. Effluent sampling for this testing shall be performed at the NPDES
Permitted final effluent discharge below all treatment processes.
All toxicity testing results required as part of this permit condition will be entered on the Effluent Discharge
Monitoring Form (MR-1) for the month in which it was performed, using the parameter code TGP3B.
Additionally, DWQ Form AT-1 (original) is to be sent to the following address:

Completed Aquatic Toxicity Test Forms shall be filed with the Environmental Sciences Section no
later than 30 days after the end of the reporting period for which the report is made.
Test data shall be complete and accurate and include all supporting chemical/physical measurements performed
in association with the toxicity tests, as well as all dose/response data. Total residual chlorine of the effluent
toxicity sample must be measured and reported if chlorine is employed for disinfection of the waste stream.
Should there be no discharge of flow from the facility during a month in which toxicity monitoring is required, the
permittee will complete the information located at the top of the aquatic toxicity (AT) test form indicating the
facility name, permit number, pipe number, county, and the month/year of the report with the notation of "No
Flow" in the comment area of the form. The report shall be submitted to the Environmental Sciences Section at
the address cited above.
Should the permittee fail to monitor during a month in which toxicity monitoring is required, then monthly
monitoring will begin immediately. Upon submission of a valid test, this monthly test requirement will revert to
quarterly in the months specified above.
Should any test data from this monitoring requirement or tests performed by the North Carolina Division of Water
Quality indicate potential impacts to the receiving stream, this permit may be re -opened and modified to include
alternate monitoring requirements or limits.
If the Permittee monitors any pollutant more frequently then required by this permit, the results of such
monitoring shall be included in the calculation & reporting of the data submitted on the DMR & all AT Forms
submitted.
NOTE: Failure to achieve test conditions as specified in the cited document, such as minimum control
organism survival, minimum control organism reproduction, and appropriate environmental controls,
shall constitute an invalid test and will require immediate follow-up testing to be completed no later than
the last day of the month following the month of the initial monitoring.

In response to your letter dated May 20, 2009 containing a draft permit for NPDES Permit
#NC0079821 I would like to request the following modifications. With each request is a -^n
justification.
1. We request a monthly average of25.0 MGD as opposed to 18.0 MGD as listed in the draft.
2. We request that the sampling frequency for iron be reduced to once per month based on the
availability of historical data.
3. We request that ammonia nitrogen be eliminated from our sampling requirements. This
facility does not disinfect by chloramination, therefore we feel sampling for this parameter is
unnecessary. We request that Footnote #6 reads as follows: "Ammonia nitrogen monitoring
only applies if the facility chloraminates".

4. As per your conversation with our Water Treatment Superintendent, Bill Brewer, on May 29,
2009 please remove the facility restart notification A. (3). As a condition of start-up of the
renovated Thomas Plant, NC DENR Division of Public Water Supply will require us to filter
the plant to waste to prove treatability. We are concerned the facility restart notification and
requirements will be in conflict with the plant start-up requirements of NC DENR's Public
Water Supply.
5. Please see the attached photo. The current coordinates listed for the discharge point are 36' De
05' 13"N latitude and 80n 12'50"W longitude. According to Google Earth the actual location
of the discharge is at 360 5'13.16"N latitude and 80°13'5.40"W longitude.

Dear Mrs. Sprinkle,
The City of Winston-Salem formally requests renewal ofNPDES Permit #NC0079821 at
our R.A. Thomas Water Treatment Plant. As requested and required this letter serves as
notification of our intention to renew this permit.
On October 20, 2008 the Thomas WTP stopped operation and Pizzagalli Construction
Company began demolition of the existing plant. This renovation project is scheduled to
be completed by December 2012 at which time the plant will be put back into service.
The attached plant diagram represents the renovated facilities. In addition, 1 have
attached an aerial photo of the Thomas Plant as it existed in October 2008 before
demolition of the plant commenced. We wish to renew this permit even though we will
not have a discharge at this outfall until the plant returns to service. Our current plan is to
handle the residuals generated by the facility in the same manner as outlined by the
correspondence with the State dated November 1, 1978.

Thomas Water Treatment Plant Solids Handling Plan
The Thomas Water Treatment Plant is an 18 MGD water treatment facility. Solids are generated
by automatic sludge removal from sedimentation basins, backwashing filters, and filter -to -waste
after filter backwashes are completed. Solids produced from filter backwashes and filter -to -waste
is routed to an equalization basin (0.545 MG), is treated with polymer, and then pumped to a
clarifier. Solids settled in the clarifier flow to the plants waste station and are then pumped to
one of two residuals lagoons (1.76 MG each) using one of three 0.288 MGD pumps. One lagoon
is kept in service while the second is dried and cleaned. Clarified water from the clarifier is
discharged to an unnamed tributary of Salem Creek through the permitted NPDES discharge
NCO079821.
Permits and Classifications:
1. The discharge from the clarifier is authorized by NPDES Permit NCO079821. The
classification for the discharge is Grade I Physical/Chemical.
Operation:
1. The system consists of a 0.545 MG equalization basin (EQ basin), (4) equalization basin
discharge pumps rated at 1.0 MGD each, a residuals clarifier rated at maximum weir loading
rate of 7.24 gpm/ft, a residuals pump station with (3) residuals pumps rated at 0.29 MGD
each for transferring residuals to (2) residuals lagoons rated at 1.76 MG each.
2. Backwash and filter to waste water from the plant process are sent to the EQ basin and then
pumped by the EQ basin pumps to the washwater clarifier. Prior to the wastewater clarifier,
the wastewater is treated with polymer to aid in clarification.
3. All sedimentation basin blow down residuals and residuals from the washwater clarifier flow
directly to the residuals pump station and are then pumped directly to one of the two
residuals lagoons.
4. Clarified water from the washwater clarifier is discharged to an unnamed tributary of Salem
Creek through the permitted NPDES discharge NCO079821.
5. Residuals are removed from the lagoons and hauled to an application site adjacent to the
plant by a private contractor.
6. Once the sludge has dried it is spread and a grass cover planted to stabilize the area.
Application Requirements:
1. Bulk residuals are not applied under the following conditions:
i. If the residuals are likely to adversely affect a threatened or endangered species.
ii. If the application causes prolonged nuisance conditions.
iii. If the land fails to assimilate the bulk residuals or the application causes the
contravention of surface water or groundwater standards.

iv. If the land is flooded, frozen, or snow-covered or is otherwise in a condition such that
runoff of the residuals would occur.
v. During a precipitation event or within 24 hours following a rainfall event of 0.5 inches
or greater in a 24-hour period.
vi. For residuals treatment and storage facilities, the following minimum setbacks shall be

3. Specific bulk residuals land application area boundaries are clearly marked on each land
application site prior to and during a residuals land application event.
Spills and Noncompliance Notification:
1. The Winston-Salem Regional Office, telephone number (336) 771-5000 will be notified
within 24 hours or the next working day following the occurrence or first knowledge of the
occurrence of any of the following:
i. Any occurrence with the distribution program which results in the land application of
significant amounts of wastes which are abnormal in quantity or characteristic.

ii. Any failure of the distribution program resulting in a release of materials to receiving
waters.
iii. Any process unit failure, due to known or unknown reasons, that render the facility
incapable of adequate residual treatment.
2. Persons reporting such occurrences by telephone shall also file a written report in letter form
within (5) days following first knowledge of the occurrence. This report must outline the
actions taken or proposed to be taken to ensure that the problem does not recur.
