Additional information is required before we may continue our review. Please address and submit the following item(s) within 30 (thirty) days of receipt of this letter: 1. Missing Waste Utilization Plan: Our record show either a portion of or entire copy of Waste Utilization plan (WUP) or nutrient management plan (NMP) is missing with your permit application. One or more of the following components is missing: a. The method by which waste is applied to the disposal fields (e.g. irrigation, injection, etc.) b. A map of every field used for land application (for example: irrigation map) c. The soil series present on every land application field d. The crops grown on every land application field e. The Realistic Yield Expectation (RYE) for every crop shown in the WUP f. The maximum PAN to be applied to every land application field g. The waste application windows for every crop utilized in the WUP h. The required NRCS Standard specifications 2. Missing Copies of Site Map/Field Maps: Our record show that the copies of the field maps are missing in your WUP or NMP. 3. Emergency Action Plan 4. Insect Control Checklist with chosen best management practices noted 5. Odor Control Checklist with chosen best management practices noted 6. Lagoon/storage pond capacity documentation (design, calculations, etc.) Also provide any site evaluations, wetland determinations, or hazard classifications that may be applicable to your facility. 8. Operation and Maintenance Plan 9. Change in animal numbers: The animal numbers are different in your application and WUP than the permitted capacity. Please be sure to revise your application or WUP to reflect the permitted capacity. If it is a change in operation type, be sure to not exceed the permitted steady state live weight. 10. A statement regarding your future intended plans for the facility. This statement should indicate whether you intend to repopulate the facility in the future and when you expect that might occur. If you do not intend to repopulate the facility, you should indicate a proposed plan for the closure of the waste storage structures. Please reference the subject application number when providing the requested information. All revised and/or additional documentation shall be signed, dated and sent to my attention at the address below.
