accom panied this attached letter. The enclosed Certificate of Approval m ust be posted at the job
site. This plan approval shall expire three (3) years follow hg the date of approval, if no land-
disturbing activity has been undertaken, as is required by Title 15A NCAC 4B .0129.
As of April 1, 2019, all new construction activities not explicitly exem pt are required to corn plete
and subm t an electronic Notice of Intent (eNOI) form requesting a Certificate of Coverage (COC)
under the NCG010000 Construction General Perm I. After the form is review Ed and found to be
corn plete, you w 11 receive a link w th paym ant instructions for the annual perm it fee. After the fee
is processed,you w Il receive the COC. As the Financially Responsible Party show non the FRO form
subm tted for this project,you M LIST obtain the COC prior to com m eicem Ent of any land disturbing
activity. The eNOI form may be accessed at deq.nc.gov/NCG01.

Please direct questions about the eNOI form to the Storm w 2ter Program staff in the Raleigh central
office. If the ow rer/operator of this project changes in the future, the new responsible party m ust
obtain a new COC.
Title 15A NCAC 4B .0118(a) and the NCG01 perm it require that the follow hg documentation be
kept on file at the job site:
1. 'Ihe approved E&SC plan as w dl as any approved deviation.
2. 1he NCG01 perm it and the COC, once it is received.
3. records of inspections m ale during the previous 12 m anths.
Also, this letter gives the notice required by G.S. 113A-61.1(a) of our right of periodic inspection to
ensure compliance w th the approved plan.
North Carolina's Sedim citation Pollution Control Act is perform aice-oriented,requiring protection
of existing natural resources and adjoining properties. If, follow hg the com m Encem Ent of this
project, the erosion and sedim citation control plan is inadequate to meet the requirem ents of the
Sedim citation Pollution Control Act of 1973 (North Carolina General Statute 113A-51 through 66),
this office may require revisions to the plan and im plem citation of the revisions to ensure
corn pliance w th the Act.
Acceptance and approval of this plan is conditioned upon your corn chance w th Federal and State
w aer quality laws, regulations, and rules. In addition, local city or county ordinances or rules may
also apply to this land-disturbing activity. This approval does not supersede any other perm it or
approval.
Please note that this approval is based in part on the accuracy of the inform lion provided in the
Financial Responsibility Form and on the plan, w lich you provided. You are requested to file an
am ended form if there is any change in the inform&ion included on the form.
