This review reflected compliance with the subject permit. The following item of concern was noted: • Per Permit Condition 111.3, a land application notification is required at least one business day in advance of conducting any land application notification activity. A review of available records shows the Winston Salem Regional Office did not receive a land application notification for field CS-14 prior to application at that field. Moving forward, be sure to notify this office at least one day before land application commences. Please refer to the enclosed inspection report for additional comments and observations. Four sample events were completed prior to land application to confirm residuals were compliant with all permit limits. • Fecal and 30 day bench were used to meet pathogen and vector reduction requirements. Fecal samples were collected on 4/3/23, 4/18/23, 5/1/23, 5/15/23. 30 day bench samples were collected 5/23/23 (x2) and 6/1/23 (x2). • Nutrient and metal samples were collected on 4/3/23, 4/18/23, 5/1/23, 5/15/23. TCLP sample was collected on 4/3/23. • The pathogen and vector attraction reduction form for the 5/1/23 sample event reported incorrect fecal values. Mr. Brent Collins is submitting an amended copy of the annual report with the corrected form. The fecal results provided for this sample event reflect compliance with the subject permit.
