Describe:
7. Has sedimentation damage occurred since last inspection? Yes
Lake/Natural watercourse off the tract
If Yes, where? (check all that apply)
Description: Off-site sediment observed in W-02 at STA 207+00 & W-04 at STA 218+00.
Degree of damage: Slight
8. Contact made with (name): George Brodeur Title: Haskell
Inspection report: Sent Report Date given/sent: May 07, 2024
9. Corrective action needed:
1. Sediment impacts to the wetlands identified as Impact W-04 (STA 218+00) and Impact W-02
(STA 207+00) must be removed. It is recommended that you use hand labor (buckets, shovels,
and wheelbarrows) to remove deposited sediment from the wetland area down to the natural
substrate channel and stabilize the impacted area with a native seed mix.
Please ensure to copy the Division of Water Resources (zachary.thomas@deq.nc.gov and
sue.homewood@deq.nc.gov) on all responses pertaining to sediment removal and restoration
efforts.
2. Slight sediment deposition was observed at Impact S-02 (STA -220+00) and the eastern boundary
of Impact W-04 (STA 219+00). The accumulation appears slight enough that sediment removal is
not required in these areas, however, the impacted areas should be stabilized with a native seed
mix and erosion control measures (silt fencing and wattles) should be improved to prevent further
sediment loss into wetlands and surface waters.

3. Install and maintain silt fence outlets per the approved plan details; with 12-inches of 57-stone
height against the hardwire mesh. Rip rap check dams are not appropriate replacements for silt
fence outlets. Routinely remove accumulated sediments when the deposition reaches 50% of the
outlet stone height to prevent sediment loss.
4. Continue to maintain erosion control measures around all active and completed wetland/stream
crossings until permanent ground cover is established to prevent sediment damage from
occurring to surface waters or wetlands. Please note, upon removal of temporary crossings, the
DWR reserves the right to request further restoration of the area pending the current conditions.
