Mike & Pine Level Staff,
This application is being withdrawn since it's been well -over the 30 days that additional information should be provided
upon request (the application was received in August and additional info was originally requested Sept 12t"), and a
response was not received to my last email on December 5t" requesting confirmation that the additional info would be
provided that week (see all emails below).
If a permit is needed in the future, a new application and accompanying documentation and fee should be submitted.

We're following -up on this sewer extension application. We (DWR) originally requested additional information on
September 12, 2022, and have followed -up several times since (see emails below). The 2T Rules state that applications
should be returned if additional information is not provided within 30 days.
Please let us know if you plan to provide the additional information this week. If not, the application will be returned.

Jason, We are waiting on the agreement to be finalized between Pine Level and the county. We have been told by the
mayor that they are working on finalizing that agreement. If memory recalls correctly, Pine Level was going to have it on
the October meeting agenda. Once they finalized the agreement, we/town/county can finalize the FTSE.

During the review of the sewer extension application for Emily Gardens Phase I, the following questions have come up:
1. Section V1.2 states an answer of 'yes' to applicant is developer and include DOA. Since the applicant is listed as
Pine Level (municipality), please confirm that the Town will be the applicant/permittee and that the answer
should be 'no' and was marked yes in error.

2. The provided USE has been signed by Jeff Holt, Pine Level Mayor, and not by a representative of the receiving
treatment system indicated as Central Johnston County Regional WWF (NC0030716). Please confirm the
receiving facility and submit an updated USE form that includes a signature from a representative of Johnston
County to indicate notification/acceptance of this additional flow.
3. We would like to confirm that there are no downstream pump stations that this new extension will be flowing
into.
