Mrs. High,
Thank you very much! You should get a confirmation email when we receive your application at the
office which will also list your permit reviewer (it may not be me, since we have 5 other reviewers in the
office). That person will review your application and contact you if they have any additional questions. If
we don't have any further questions, then we'll write a draft permit and send it to you for you to ask
questions or make comments on the new permit. Assuming you accept the draft, we'll then issue a new
permit shortly afterwards, and mail you a hard copy.

Hello Ms. Hannum,
I received the permit today, it was postmarked 4/3. 1 will make sure it is at our post office tomorrow
before the 2pm pickup. I honestly didn't remember anything about a permit renewal until I received your
email.
Thanks for your help with this!
Sincerely,
Marshia High

I'll get a copy sent out to you today. I know many libraries often have printers and scanners you can use
either for free or for a few quarters, if that helps in the future!

Unfortunately, I don't have access to a printer. If it won't cause a problem for us, please mail me a hard
copy and I will return it immediately. I will also try to figure out how to complete the form online and
send it back.. I really need to learn this crap!!

Mrs. High,
It looks like the one we received through the other email address also didn't have any signatures. Can
you print out the application and sign it, then either scan it back into the computer or mail us a
physical copy?
If you don't have access to a printer, I can mail you a hard copy of the application.

Okay, we've just checked that email address and can confirm that we received your application at 2
PM. We typically check that email address once a day.
You should receive a confirmation email within the next few days listing who your permit reviewer will
be (it may not be me, since we have 5 other permit reviewers in the office). They'll contact you if they
have any additional questions. Thanks, and have a great rest of the week!

I sent it to the main Non -Discharge. Report email address. I really don't know much about filling in
forms online and sending back the completed forms. If you don't have it please mail me a copy and I
will return it immediately. Either way, please let me know what to expect.

The attachment to your previous email did not have any signatures on pages 2 or 4. Did you submit a
signed application to a different email address, or perhaps send it in via regular mail?

Dear Mr. and Mrs. High,
Division of Water Resources records indicate that your wastewater irrigation permit, which was
issued March 27, 2018, expires April 30, 2023. To date, the Division has not received a renewal
application. It is a condition of your permit and required by administrative code 15A NCAC 02T .0109
to request renewal of your permit at least 180 days prior to its expiration.
As the Permittee, you are responsible for renewing and maintaining this permit. Failure to do so may
result in assessment of civil penalties in accordance with North Carolina General Statute §143-
215.6A. Therefore, it is important that you respond as soon as possible by selecting one of the
following options:
1. APPLY FOR PERMIT RENEWAL —A renewal will allow the operation of your non -discharge
system to continue. You will be expected to pay any annual fees and follow all operation
and maintenance procedures specified in your new permit. You may renew your permit by
completing and submitting the attached renewal application. I have pre -filled as much
information as I could from our records; please complete the sections that are not filled out
6
and double check everything for accuracy. If you need a hard copy of the form, let us know
and we will mail one to you.
2. APPLY FOR PERMIT RESCISSION -If the non -discharge system has not been constructed or
if it is no longer active, you may request to rescind your permit by replying to this email. By
rescinding your permit, all overdue annual fees will be waived; however, if you wish to
operate the non -discharge system in the future, you will have to apply for and receive anew
permit.
3. CHANGE OF OWNERSHIP -If you no longer own the property containing the subject
facilities, please have the new owners contact us to request a Change of Ownership
application.
Please submit the completed forms electronically via one of the following methods:
1. Return via email tome or to Non-Discharge.Reports@ncdenr.gov.
OR
2. Mail a hard copy to:
Non -Discharge Branch
1617 Mail Service Center
Raleigh, NC 27699-1617
