Unfortunately our database doesn't always update everything at once, so it's possible that your billing address got updated but not the correspondence address. I couldn't tell which was the most recent address on my end, so I think I just picked one for the letter. We'll make sure to get that updated across the board during the renewal process. Alys, Attached is the Word document that has been updated with our correct mailing address and a PDF file that contains scans of the 2 pages requiring our signatures. I sincerely apologize for being late and appreciate your diligence in following up. Somehow I missed the original correspondence back in October. Perhaps it is because the document you sent me has an old mailing address for our point of contact rather than the property where we now reside. I have updated that in the Word document. I'm not sure how that reverted because we have always received the annual notices at the correct address. Regardless, please make sure that our address is updated appropriately in the system. Wastewater Irrigation - WQ0014100 Dear Mr. and Mrs. Allen, Division of Water Resources records indicate that your wastewater irrigation permit, which was issued June 22, 2017, expired October 31, 2022. To date, the Division has not received a renewal application. It is a condition of your permit and required by administrative code 15A NCAC 02T .0109 to request renewal of your permit at least 180 days prior to its expiration. As the Permittee, you are responsible for renewing and maintaining this permit. Failure to do so may result in assessment of civil penalties in accordance with North Carolina General Statute §143-215.6A. Therefore, it is important that you respond as soon as possible by selecting one of the following options: APPLY FOR PERMIT RENEWAL — A renewal will allow the operation of your non -discharge system to continue. You will be expected to pay any annual fees and follow all operation and maintenance procedures specified in your new permit. You may renew your permit by completing and submitting the attached renewal application. I have pre -filled as much information as I could from our records; please complete the sections that are not filled out and double check everything for accuracy. If you need a hard copy of the form, let us know and we will mail one to you. 2. APPLY FOR PERMIT RESCISSION — If the non -discharge system has not been constructed or if it is no longer active, you may request to rescind your permit by replying to this email. By rescinding your permit, all overdue annual fees will be waived; however, if you wish to operate the non -discharge system in the future, you will have to apply for and receive a new permit. 3. CHANGE OF OWNERSHIP —If you no longer own the property containing the subject facilities, please have the new owners contact us to request a Change of Ownership application.
