Inspection Summary:
Kelly Jonas and Melissa Joyner met with Mr.Avery Hildreth, Batch Operator, to conduct an industrial stormwater plan
technical assistance at the Hildreth Redi-Mix facility in Rockingham, Richmond County, NC.The Stormwater Pollution
Prevention Plan (SWPPP)was reviewed per the conditions of the National Pollutant Discharge Elimination System (NPDES)
General Permit NCG140000.
The facility grounds were inspected, including the areas indicated where the four outfalls are located and the retention basin.
Outfalls 3 and 2 flow within the property line. Outfalls 1 and 4 do not appear to have a point source discharge, but rather are
areas of sheetflow.The majority of stormwater flows to: the retention basin, a grassy depressed area near Hwy 74 Bus on
the south side of the property, and a grassy ditch on the northeast side of the property. Minimal stormwater actually flows
offsite.
After the site visit, Mrs. Melissa Hildreth, owner,was contacted. Mrs. Hildreth indicated her husband applied for the permit
and was more familiar with its conditions. However, Mr. Hildreth has since died.
Upon inspection of the facility, it is recommended the facility apply for a rescission of its NPDES Permit NCG140385. The
recommendation is based on the inspection of the facility and observing no point source runoff areas. The Rescission

In compliance with the provision of North Carolina General Statute 143-215.1, other lawful
standards and regulations promulgated and adopted by the North Carolina Environmental
Management Commission, and the Federal Water Pollution Control Act, as amended,

is hereby authorized to operate wastewater treatment system(s) and discharge
stormwater and/or wastewater (as per original approval or subsequent modifications),
from a facility located at:

to receiving waters designated as Hitchcock Creek (Midway Pond, Steeles Mill Pond), class
C waters in the Yadkin River Basin(s), in accordance with the effluent limitations,
monitoring requirements, and other conditions set forth in General Permit No. NCG140000
which can be downloaded by entering"deq.nc.gov/SW-industrial" in the browser window
and navigating to the "NPDES Stormwater General Permits" web page.
This certificate of coverage shall become effective July 1, 2022.
This Certificate of Coverage shall remain in effect for the duration of the General Permit or
until rescinded.
