Please note that the two maintenance schedule items are: (a) The renewal of the permit before the old one expires — which we are doing now (b) The closure of the system in the event a public sewage system were to become available —this has not occurred I therefore checked No in this section. Could you please double-check that? Please also make sure both you and Ms. Luz Maria sign the Operation and Maintenance Agreement on page 2, and complete the checkboxes on pages 3-4. As for your other question, this is a regular renewal cycle in addition to the annual fee, you're correct. The renewal requirement is in your current permit, which expires at the end of this year. Attached is the completed form (I corrected the zip code, all other information was correct). I paid the annual fee last month (see attached invoice), but never received a notification about this permit renewal. Is this a 4- year cycle renewal in addition to the annual fee? To date, the Division has not received a renewal application. It is a condition of your permit and required by administrative code 15A NCAC 02T .0109 to request renewal of your permit at least 180 days prior to its expiration. As the Permittee, you are responsible for renewing and maintaining this permit. Failure to do so may result in assessment of civil penalties in accordance with North Carolina General Statute §143- 215.6A. Therefore, it is important that you respond as soon as possible by selecting one of the following options: APPLY FOR PERMIT RENEWAL — When you apply for and receive a renewal, operation of your non -discharge system will be permitted to continue. As such, the payment of annual fees, and any operation and maintenance procedures specified in your permit will be required. You may renew your permit by completing and submitting the attached permit renewal application. I have pre -filled as much information as I could from our records; please double check everything for accuracy. 2. APPLY FOR PERMIT RESCISSION — If the non -discharge system has not been constructed, or if it is no longer active, you may request to rescind your permit. By rescinding your permit, all overdue annual fees will be waived; however, if in the future you wish again to operate the subject non -discharge system, you must first apply for and receive a new permit. 3. CHANGE OF OWNERSHIP — If you no longer own the property containing the subject facilities, please request a Change of Ownership application. Please submit the completed forms electronically via one of the following methods: For files less than 20 MB, send via Email to me or to Non -Discharge. Reports@ncdenr.gov. 2. For files greater than 20 MB, upload via the following link: https://edocs.deg.nc.gov/Forms/NonDischarge- Branch-Submittal-Form-Ver2 We preferthat you submit your application online, but we will accept physical mail. The only physical mail that is required is an application fee if your renewal includes a major modification. If you are not making any changes to the system, then the renewal does not include a modification, and there is no application fee. If an application fee is necessary, or to mail in your renewal form, please send the documents to my attention at 1617 Mail Service Center, Raleigh, NC 27699-1617.
