overdue annual fee. Pursuant to Permit No. WQ0040707 and 15A NCAC 02T .0105(e)(2), the Permittee
shall pay an annual fee for each year of the term of the permit.
Please remit payment electronically (https:Hdeq.nc.gov/epUments/wQ) or in the form of a check,
money order, or electronic funds transfer in the amount of $60.00 for the following overdue annual fee
(invoice enclosed). Make checks payable to the North Carolina Department of Environmental Quality.

Failure to pay outstanding annual fees within 30 calendar days of this notice shall result in further
enforcement action. Remit payment electronically or mail to the following address:

If the Permittee has not constructed the subject non -discharge system, or if it is no longer active,
the Permittee may request to have the permit rescinded. Once both the Central Office and Regional Office
approve the rescission request, the Division will waive all overdue annual fees. However, if the Permittee
desires to construct and/or operate the subject non -discharge system in the future, the Permittee must obtain
a new permit. Email Non-Discharge.Reportsgdeq.nc. og_v to request a permit rescission.

The Permittee is still responsible for the permit and annual fees, even if the Permittee no longer
owns the land associated with the non -discharge system. As noted in the permit, the subject permit is not
transferrable with the land. The new landowner must submit a Change of Ownership application and the
Division reissue the permit in their name to release the current Permittee from all annual fee responsibilities.
