Dear Permittee,
Division of Water Resources' records indicate that Permit No. WQ0041136 has an overdue annual
fee. Pursuant to 15A NCAC 02T.01 05(el(21, the administering and compliance monitoring fee
shall be paid annually. Failure to pay outstanding annual fees shall result in future Compliance
Notices.
Accordingly, please remit payment using ePayments, check, money order, or electronic funds
transfer made payable to the North Carolina Department of Environmental Quality in the amount
of $810.00 for the following overdue annual fee. Please also double-check the billing address on
the attached invoice for accuracy so we can ensure invoices are mailed to the correct place.

If the overdue annual fee was paid prior to receiving this notice, provide documentation verifying
payment (e.g., receipt, cancelled check, etc.) to this email address.
If the non -discharge system has not been constructed, or if it is no longer active, the permit may
be rescinded. By rescinding the permit, all overdue annual fees will be waived; however, if it is
desired to again construct and/or operate the subject non -discharge system, a new non -
discharge permit shall be applied for and received. Permit rescission requests may be emailed to
Non-Discharge.Reports@ncdenr.gov.
If the Permittee no longer owns the non -discharge system, please notify the Division so that we
can contact the new property owners. As noted in your permit, the subject permit is not
transferable with the land, therefore the Permittee is still responsible for the permit and annual
fees until a permit is reissued to the new landowner.
