Of course! I didn't realize it hadn't gotten forwarded. Please see attached.
I've also attached a permit renewal form since this permit is expiring at the end of January. It is a condition of
your permit and required by administrative code 15A NCAC 02T .0109 to request renewal of your permit at
least 180 days prior to its expiration.
Please submit the completed forms electronically via one of the following methods:
1. For files less than 20 MB, send via Email to me or to Non -Discharge. Reports@ncdenr.gov.
2. For files greater than 20 MB, upload via the following link:
https://edocs.deg.nc.gov/Forms/NonDischarge-Branch-Submittal-Form-Ver2

Thank you for the update; I'll remove Mr. Lockhart from our list of contacts for that facility
I also just noticed that this permit is expiring at the end of January, and we have not yet received a renewal

Dear Permittee,
Division of Water Resources' records indicate that Permit No. WQ0019331 has an overdue annual
fee. Pursuant to 15A NCAC 02T.01 05(el(2), the administering and compliance monitoring fee shall
be paid annually. Failure to pay outstanding annualfees shall result in future Compliance Notices.
Accordingly, please remit payment using ePayments, check, money order, or electronic funds transfer made
payable to the North Carolina Department of Environmental Quality in the amount of $1,310.00 for the following

If the overdue annual fee was paid prior to receiving this notice, provide documentation verifying payment (e.g.,
receipt, cancelled check, etc.) to this email address.
If the non -discharge system has not been constructed, or if it is no longer active, the permit may be rescinded. By
rescinding the permit, all overdue annual fees will be waived; however, if it is desired to again construct and/or
operate the subject non -discharge system, a new non -discharge permit shall be applied for and received. Permit
rescission requests may be emailed to Non-Discharge.Reports@ncdenr.gov.
If the Permittee no longer owns the non -discharge system, please notify the Division so that we can contact the
new property owners. As noted in your permit, the subject permit is not transferable with the land, therefore the
Permittee is still responsible for the permit and annual fees until a permit is reissued to the new landowner.
