From: Herndon, Mason Sent: Friday, January 15, 2016 10:16 AM To: Mountcastle, Michael D; Shaver, Brad E SAW; Williams, Priscilla T Cc: Richie Brown; Mathis, Stonewall D; Carpenter,Kristi Subject: RE: NCDOT Burgaw Multi-Unit Office Importance: High Mr. Mountcastle, DWR has reviewed the information you submitted on January 13, 2016 and found the application to be incomplete and cannot process your request as submitted. One of the issues was that we did not receive a complete PCN document (we received Page 4 only). Brad Shaver with the Corps of Engineers forwarded us a copy of the complete PCN document, as well as other required information so that issue has been resolved. However, after reviewing the permit drawings and the revised PCN page 4 that you did submit to us, we found that the impact numbers on the permit drawing did match the impact numbers in the PCN. This was discussed with Mr. Shaver this morning and was the reason for his e-mail below. I have taken the liberty of revising Page 4 of the PCN (attached) to match the impact numbers on the permit drawing that was submitted. I also made some clarifications in Section 3i of this sheet. Please review the attached PCN page 4 and confirm that the impact numbers and information in Section 3i are consistent with your request. If you agree with the revisions please initial, date and e-mail this sheet back to me. Also, there is a$240 dollar permit application fee associated with this request. DWR needs a WBS# from which we can debit this fee. Please provide this number in your response. Once we receive this information we will consider this a complete application and can begin processing your request. If you have questions or need any additional clarifications, please do not hesitate to contact me. First off I miss calculated bank stabilization as 32' not 42' but more importantly I wanted to be clear that the total new permanent channel impacts total 199'. That breakdown is 3' culvert extension, 180' of channel restoration which includes 26' of bank stabilization on the outlet, and 16' of bank stabilization on the inlet side. Sorry for the miscue but the current authorization covers the desired activity.
