In compliance with the provision of North Carolina General Statute 143-215.1, other lawful standards and
regulations promulgated and adopted by the North Carolina Environmental Management Commission, and
the Federal Water Pollution Control Act, as amended,

is hereby authorized to discharge stormwater from a site located at:

8401 Planer Mill Rd
Middlesex
Nash County
to receiving waters designated as Turkey Creek, class C;NSW waters in the Neuse River Basin(s), in
accordance with the effluent limitations, monitoring requirements, and other conditions set forth in N.C.
General Permit No. NCG210000, issued on 5/30/2023 and effective on 6/1/2023.
This Certificate of Coverage shall become effective on 5/6/2024.
This Certificate of Coverage shall remain in effect for the duration of the General Permit or until rescinded.

Owed fees were paid after mass renewal for NCG210000 General Permit in 2023, but this COC was not
renewed. Permit coverage is active again (please see attached screen shot from our system) and reflect
current owner/operator name. The Permit Summary Report should refresh in 24-48 hours with the Active
Status.
