Stokes County
Division personnel have reviewed and approved your application for renewal of the subject permit.
Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued pursuant
to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between North Carolina and the U.S. Environmental Protection Agency dated May 9, 1994 (or as
subsequently amended).
Please note that Total Residual Chlorine monitoring has been changed to 2/Week and Temperature
monitoring has been changed to daily. These changes were made in accordance with the 213.0500
Rules for class II facilities. (Refer to A. (1.).)
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you. you have the right to an adjudicatory hearing upon written request within thirty
(30) days following receipt of this letter. This request must be in the form of a written petition,
conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of
Administrative Hearings (6714 Mail Service Center, Raleigh, North Carolina 27699-6714). Unless
such demand is made, this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may
require modification or revocation and reissuance of the permit. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or
permits required by the Division of Land Resources. the Coastal Area Management Act or any other
Federal or Local governmental permit that may be required.

You called and asked about deleting the removal efficiencies for TSS and BODS from the final permit. These
are federal requirements and cannot be removed from the permit. Just wanted to let you know that the final
permit will be issued exactly like the draft, with 2 exceptions .... TRC will be changed to 2/Week and Effluent
Temperature will be changed to daily. (per 2B.0500 rules for class II facilities). Please let me know if you have
any questions.

Stokes County Wastewater Treatment Plant
Narrative Description
Wastewater is pumped from Mill Creek Lift Station into the equalization basin.
The wastewater is then pumped at approximately 30 gpm into the aeration basins.
Each basin receives 15 gpm. Wastewater flows into the clarifier where the sludge
settles and is returned at 60 to 80 gpm to the aeration basin. The clarifier effluent
flows over the weirs passing through the flowmeter and the ultra violet disinfection
system. From this point the effluent flows into an on -site lift station that conveys
the effluent to a discharge point on the Dan River approximately 1/4 of a mile
away.
Danbury Wastewater Treatment Plant
Sludge Management Plan
Sludge is wasted to an aerobic digester where it is digested and clear water is
decanted. The sludge is then hauled by Carolina Septic of Walkertown, North
Carolina. The sludge is disposed of at the City of Winston-Salem's Wastewater
Treatment Plant.

I appreciate your checking into the faxed letter on this subject that I
left with you. I also have 3 more pages of the fax that I will drop off
to you. According to Pam's mail log, the original letter was sent to me
in April but I do not remember it. Either way, since it is a wetland
system being proposed for polishing of an NPDES permitted discharge, I
guess the response should come from your Unit.
In the voice mail you left me, you asked what you could do to help me on
this. If you will, please have someone call these folks and see if what
they are proposing is acceptable. The date of the letter is April so I
am sure they are starting to get very impatient.

Dear Ms. Sullivan:
The Stokes County Soil and Water Conservation District is interested in exploring the
option of creating a Constructed Wetland Demonstration Project in Danbury, North
Carolina. This project would be created by diverting a portion of the treated effluent that
currently is discharged under the above referenced permit number through a new wetland.
The constructed wetland would be located near Moratock Park which is less than 150
yards from the existing permitted discharge point.
The idea is for this portion of effluent to be diverted as necessary to keep the wetland wet
and thereby assisting in its functionality. The project would be a demonstration site and
would be used for research and education purposes. The project would be handled in
cooperation with Stokes County who is the NPDES permit holder.
The purpose of my letter to you today is to formally request guidance as to what
procedural steps would need to be taken to assure that the permit is complied with
exclusively. Again, to reiterate, we would be utilizing a sidestream of the treated effluent
(better than secondary quality) through the wetland and return it to the original discharge
point. We are pleased that we have the opportunity to provide this educational project.
We do, however, want to assure that we are starting it properly.
