The Animal Feeding Operation Program of the Division of Water Resources (Division) has completed a
preliminary review of your renewal permit application package. Additional information is required
before we may continue our review. Please address and submit the following item(s) within 30 (thirty)
days of receipt of this letter:

1. Emergency Action Plan

2. Insect Control Checklist with chosen best management practices noted

3. Odor Control Checklist with chosen best management practices noted

4. Mortality Control Checklist with selected method noted Use the enclosed updated Mortality
Control Checklist

5. Change in regulated operation type: The animal numbers and regulated operation type are
different in your application and WUP than the permitted capacity. Please be sure to revise your
application or WUP to reflect the permitted capacity. If it is a change in operation type, be sure to
not exceed the permitted steady state live weight.

Please reference the subject application number when providing the requested information. All revised
and/or additional documentation shall be signed, dated and sent to my attention at the address below. The
