The Division of Water Quality (the Division) hereby issues the attached NPDES permit for the subject
facility. This permit is issued pursuant to the requirements of North Carolina General Statute 143-215.1
and the Memorandum of Agreement between North Carolina and the U.S. Environmental Protection
Agency dated October 15, 2007, or as subsequently amended.
Based on your application to renew, the Division has made only minor changes to your previous permit.
If any parts, measurement frequencies, or sampling requirements contained in this permit are
unacceptable, you have the right to an adjudicatory hearing, upon written request submitted within thirty
(30) days after receiving this letter. Your request must take the form of a written petition conforming to
Chapter 150B of the North Carolina General Statutes, and you must file it with the office of
Administrative Hearings, 6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Unless such a
demand is made, this permit shall be final and binding.
This permit is not transferable except after notifying the Division of Water Quality. The Division may
require permit modification, or revocation and re -issuance. Please note that this permit does not affect
your legal obligation to obtain other permits required by the Division of Water Quality, the Division of
Land Resources, the Coastal Area Management Act, or other federal or local governments.

in accordance with effluent limitations, monitoring requirements, and other conditions set forth in Parts
I, II, III, and IV hereof.
This permit shall become effective December 1, 2010.
This permit and the authorization to discharge shall expire at midnight on October 31, 2014.

revoked, and as of this issuance,. any previously issued permit bearing this number is no longer
effective. Therefore, the exclusive authority to operate and discharge from this facility arises under

Footnotes:

2. All flow discharging from the system shall be monitored. Flow shall be released slowly so as to prevent
erosion of the receiving stream. Total flow from the outfall for one discharge event shall not exceed
the capacity of the storage tank.
3. Effluent shall not cause the receiving -stream turbidity to exceed 50 NTU. If natural background
turbidity exceeds this.level, this discharge shall not increase in the natural receiving -stream turbidity.
4. Effluent temperature shall not cause receiving stream temperature to increase more than 2.8°C, and in
no case shall it cause the ambient water temperature to exceed 32°C.
5. Acute Annual Toxicity (Fathead Minnow), 24-hour static test [see Section A. (2)].
The Permittee shall discharge no floating solids or foam visible in other than trace amounts.

A. (2) ACUTE TOXICITY MONITORING (ANNUAL)
The Permittee shall conduct annual toxicity tests using protocols defined as definitive in E.P.A.
Document EPA/600/4-90/027 entitled "Methods for Measuring the Acute Toxicity of Effluents to
Freshwater and Marine Organisms." The monitoring shall be performed as a Fathead Minnow
(Pimephales promelas) 24-hour static test. Effluent samples for self -monitoring purposes must be
obtained below all waste treatment.
The Permittee will conduct one test annually, with the annual period beginning in January of the
calendar year of the effective date of the permit. The annual test requirement must be performed
and reported by June 30. If no discharge occurs by June 30, notification will be made to the
Division by this date. Toxicity testing will be performed on the next discharge event for the annual
test requirement.
The parameter code for this test is TAE6C. All toxicity testing results required as part of this
permit condition will be entered on the Effluent Discharge Form (MR-1) for the month in which it
was performed, using the appropriate parameter code. Additionally, DWQ Form AT-1 (original) is
to be sent to the following address:

Test data shall be complete and accurate and include all supporting chemical/physical
measurements performed in association with the toxicity tests, as well as all dose/response data.
Total residual chlorine of the effluent toxicity sample must be measured and reported if chlorine
is employed for disinfection of the waste stream.
Should any test data from either these monitoring requirements or tests performed by the North
Carolina Division of Water Quality indicate potential impacts to the receiving stream, this permit
may be re -opened and modified to include alternate monitoring requirements or limits.
NOTE: Failure to achieve test conditions as specified in the cited document, such as minimum
control organism survival and appropriate environmental controls, shall constitute an invalid test
and will require immediate follow-up testing to be completed no later than the last day of the
month following the month of the initial monitoring.

the discharge of wastewater from our facility located at 620 Facet Road, Henderson.
This cover letter and the enclosed application form and map are submitted as our
application package. As requested, the original and 2 copies are enclosed. There
have been no facility changes since the last permit was issued in December 2004.
A sludge management plan is not applicable to this wastewater treatment since no
sludge or solids are generated. The water is used only for cooling and the only
treatment is temporary storage for temperature control.

7. Describe the treatment system List all installed waste treatment components with capacities,
describe the processes that generate wastewaters. If the space provided is not sufficient attach a

The treatment system consists of a 500,000 gallon aboveground holding tank, which is
discharged via Outfall 001 into an unnamed tributary to Martin Creek.
Whenever molten glass is drained from a glass -melting furnace, clean city water is used
to cool the glass and the metal chute, and direct the flow of glass down the chute. The
quench water is collected and held in the 500,000 gallon aboveground storage tank for
temperature control then is discharged to the receiving stream at a controlled rate, over
a period of several days. This cooling, or quench, water does not come into contact with
any oils or other substances used in the plant, and there are no treatment chemicals or
other additives used.
The discharge is intermittent and only occurs as a result of a furnace drain event. The
primary reasons for a drain are a scheduled or emergency furnace repair. Repairs are
typically scheduled about every 7 to 10 years.

Briefly describe the manufacturing process[es]:
The raw materials are combined into batches and delivered to the two glass -melting furnaces
where they are heated to molten glass. The molten glass is then directed to a forming machine
where is it moulded into glass containers, bottles or jars. The containers are slowly heated and
cooled in annealing lehrs. The containers are inspected for quality and then packed in cartons,
or on bulk pallets. The containers are stored in an on -site warehouse until loaded in a truck for
shipment to the customer.
