On April 11, 2024, Matthew Gigante from the Mooresville Regional Office of the Division of Energy,
Mineral and Land Resources (DEMLR), conducted a site inspection for the Piedmont Hardwood Lumber
Co Inc facility located at 9000 State Hwy 49, Cabarrus County,North Carolina. A copy of the Compliance
Inspection Report is enclosed for your review. The site visit and file review revealed that the subject facility
is covered by NPDES Stormwater General Permit NCG210000 under Certificate of Coverage
NCG210419. Permit coverage authorizes the discharge of stormwater from the facility to receiving waters
designated as Dutch Buffalo Creek, class C waters in the Yadkin Pee-Dee River Basin.
As a result of the site inspection, the following permit conditions violation(s) are noted:
1) Stormwater Pollution Prevention Plan (SPPP)
Per Part B...A Stormwater Pollution Prevention Plan (SPPP) has not been developed or properly
implemented.
2) Oualitative Monitorin¢
Per Part D...Qualitative monitoring has not been conducted and/or recorded in accordance with permit
requirements.
3) Analytical Monitoring
Per Part E...Analytical monitoring has not been conducted and/or recorded in accordance with permit
requirements.

You are asked to respond to this office, in writing, within 30 calendar days from receipt of this notice.
Your written response should include a reasonable explanation as to why the aforementioned violation(s)
have occurred as well as a Plan of Action to prevent these violation(s) from recurring,
Action Items
- Within thirty Days, send an up-to-date copy of the stormwater pollution prevention plan to the
Mooresville Regional Office.
- Immediately begin analytical and qualitative monitoring per permit requirements and upload
monitoring data to the e-DMR system. If analytical benchmarks are exceeded, notify NCDEQ as
required and begin Tier One response as outlined in the permit.
Thank you for your attention to this matter. This office requires that the violations, as detailed above,
be properly resolved. These violations and any future violations are subject to a civil penalty
assessment of up to $25,000 per day for each violation. Should you have any questions regarding these

Inspection Summary:
An inspection was conducted as part of an investigation into a complaint issued on the site. No portion of the stormwater
pollution prevention plan was present during the inspection.
Analytical and qualitative monitoring is not being completed per permit requirements. No qualitative monitoring was available
on site at the time of the inspection. Some analytical monitoring data was present. Based on what was available, analytical
monitoring is not being completed per permit requirements. The most recent analytical monitoring report available was from
2023.Additionally, analytical monitoring samples reviewed during the inspection indicated exceedances of benchmark
values which have not been reported to NCDEQ.
Copies of the general permit and certificate of coverage were not available on site at the time of the inspection.
One outfall was viewed on the south side of the site near a covered storage area.The slope below the outfall was eroding.
Based on the inspection conducted, the facility was not compliant. The facility must:
-Within thirty days, send an up-to-late copy of the stormwater pollution prevention plan to the Mooresville Regional Office.
- Immediately begin analytical and qualitative monitoring per permit requirements and upload monitoring data to the a-DMR
system. If analytical monitoring benchmarks are exceeded, notify NCDEQ as required and begin Ter One response as
outlined in the permit.
