The Division of Water Quality (the Division) has reviewed and approved your application to
renew the subject permit. We issues the attached NPDES discharge permit pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between North Carolina and the U.S. Environmental Protection Agency dated May 9, 1994 (or as
subsequently amended).
The Division understands that you have made no significant alterations to your existing treatment
facilities. However, we have made minor changes to your permit to include upgrades to your
facility description and facility map. In addition we have added parameter codes to your permit
effluent page for your convenience [see permit § A. (L)]. However, please note that your database
for the parameter trichloroethylene (TCE) shows reasonable potential to exceed the North
Carolina receiving -stream Human Health Standard of 30µg/L.
Revised Limits and Monitorin>? - Based on our program -mandatory Reasonable Potential Analysis
(RPA), we have lowered your permit limit for TCE from 94.2µg/L to 30µg/L. We have also
increased the TCE monitoring frequency from Quarterly to Monthly, however we have footnoted
this parameter to provide you the opportunity to petition the Division. After sampling for one year
(minimum 12 samples) you may submit a request to reduce this monitoring frequency, as data may
warrant [see footnote, § A. (1.)].We have also increased pH monitoring frequency from Quarterly
to Monthly. Finally, the Division has reinstated the periodic effluent spot-check for volatile
organic compounds (analytical method EPA 601), to be resumed Annually.
Please be reminded that North Carolina State Regulations require you to report, on or attached to
your monthly Discharge Monitoring Report (DMR), any and all additional treatment -system
sampling and analyses conducted, even if conducted beyond your effluent -page monitoring
requirements [NCAC 2B.0506 (b) (3) (J)].

unacceptable to you, you have the right to an adjudicatory hearing upon written request within
thirty (30) days following receipt of this letter. This request must be in the form of a written
petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the

A. (2.) CHRONIC TOXICITY PERMIT LIMIT - (QUARTERLY)
The effluent discharge shall at no time exhibit observable inhibition of reproduction or significant mortality
to Ceriodaphnia dubia at an effluent concentration of 90%.
The permit holder shall perform at a minimum, guarterlX monitoring using test procedures outlined in the
"North Carolina Ceriodaphnia Chronic Effluent Bioassay Procedure," Revised February 1998, or
subsequent versions or "North Carolina Phase II Chronic Whole Effluent Toxicity Test Procedure"
(Revised -February 1998) or subsequent versions. The tests will be performed during the months of
February, May, August and November. Effluent sampling for this testing shall be performed at the
NPDES permitted final effluent discharge below all treatment processes.
If the test procedure, performed as the first test of any single quarter, results in a failure or ChV below the
permit limit, then multiple -concentration testing shall be performed at a minimum, in each of the two
following months as described in "North Carolina Phase II Chronic Whole Effluent Toxicity Test
Procedure" (Revised -February 1998) or subsequent versions.
The chronic value for multiple concentration tests will be determined using the geometric mean of the
highest concentration having no detectable impairment of reproduction or survival and the lowest
concentration that does have a detectable impairment of reproduction or survival. The definition of
"detectable impairment," collection methods, exposure regimes, and further statistical methods are specified
in the "North Carolina Phase II Chronic Whole Effluent Toxicity Test Procedure" (Revised -February 1998)
or subsequent versions.
All toxicity testing results required as part of this permit condition will be entered on the Effluent Discharge
Monitoring Form (MR-1) for the months in which tests were performed, using the parameter code TGP3B
for the pass/fail results and THP3B for the Chronic Value. Additionally, DWQ Form AT-3 (original) is to
be sent to the following address:

Completed Aquatic Toxicity Test Forms shall be filed with the Environmental Sciences Section no later than
30 days after the end of the reporting period for which the report is made. Test data shall be complete,
accurate, include all supporting chemical/physical measurements and all concentration/response data, and be
certified by laboratory supervisor and ORC or approved designate signature. Total residual chlorine of the
effluent toxicity sample must be measured and reported if chlorine is employed for disinfection of the waste
stream.
Should there be no discharge of flow from the facility during a month in which toxicity monitoring is
required, the Permittee will complete tb+e information located at the top of the aquatic toxicity (AT) test form
indicating the facility name, permit number, pipe number, county, and the month/year of the report with the
notation of "No Flow" in the comment area of the form. The report shall be submitted to the Environmental
Sciences Section at the address cited above. Should the Permittee fail to monitor during a month in which
toxicity monitoring is required, monitoring will be required during the following month.
Should any test data from this monitoring requirement or tests performed by the North Carolina Division of
Water Quality indicate potential impacts to the receiving stream, this permit may be re -opened and modified
to include alternate monitoring requirements or limits.
NOTE: Failure to achieve test conditions as specified in the cited document, such as minimum control
organism survival, minimum control organism reproduction, and appropriate environmental controls, shall
constitute an invalid test and will require immediate follow-up testing to be completed no later than the last
day of the month following the month of the initial monitoring.

CORR Environmental Resources Inc. (CORR) as an authorized representative of AAF-
McQuay Inc (AM]) is responding to the February 15, 2012 Draft NPDES permit renewal
application referenced above. The Draft Permit will regulate the effluent discharge
resulting from the groundwater remediation operation at the former Heatcraft property
located at 602 Sunnyvale Drive in Wilmington, New Hanover County. The Draft Permit
was received by CORR on February 23, 2012. CORR offers the following response to
the Draft Permit:
• Agree to requirement of 30 pg/L (ppb) for trichloroethene (TCE) for the effluent
discharge;
• Agree to monthly sampling for a period of one year (12 samplings), at which time
AMI can petition the Branch for reduced sample frequency;
• Agree to annual testing for volatile organic compounds using EPA Method 601;
• Agree to the quarterly monitoring measurement frequency requirement for
Chronic Toxicity Testing; and
• Agree to the monthly average discharge flow limit of 0.36 Million Gallons per Day
(MGD) from outfall 001.
The pH requirement of 6 to 9 Standard Units will also be tested in conjunction with the
monthly monitoring requirement. The monthly monitoring for TCE will be subcontracted
to a North Carolina certified laboratory and will utilize standard EPA Method 8260 for
volatile organic compounds (VOC's). Please note the monthly sample results will
provide the full analytical report as an attachment for all VOC constituents on the Method
8260 list as has been provided in the past.
The Chronic Toxicity testing will also be subcontracted to a North Carolina certified
laboratory. The Chronic Toxicity testing will use the referenced "North Carolina
Ceriodaphnia Dubia Chronic Effluent Bioassay Procedure," or the "North Carolina Phase
II Chronic Whole Effluent Toxicity (WET) Test Procedure," both revised February 1998.

One other comment is offered; the Supplement to Permit Cover Sheet in Section 1, last
bullet states "discharge line to a concrete ditch". The effluent discharge piping exits the
water treatment plant and courses to the south along the western fenced perimeter
where it discharges to the ditch which separates the former Heatcraft property from the
adjoining private industrial property. This ditch is not concrete lined but consists of
native soils. Upstream of the outfall, flow is intermittent and does not typically constitute
a natural stream but provides for storm water runoff. This ditch then flows to the east
under Sunnyvale Drive via a culvert where it then travels parallel to Sunnyvale Drive for
several hundred yards to the south before meandering to the undeveloped properties to
the east.
No other comments are offered concerning the Draft Permit as received and CORR
recommends issuance of the Final Permit as stated in your cover letter to the Draft
Permit.

Background — DWQ's authorizations to construct and operate groundwater -treatment at this facility
date from 1994, but there have been multiple property owners (at least six) since that time. Chlorinated
solvents were stored at the former HeatCraft plant and used to degrease parts during the manufacture
of refrigeration coils. Adverse environmental impacts date from 1983, with a first spill of 400 to 500
gallons of trichloroethylene (TCE), and a second release 1987 of 55 gallons of trichloroethane (TCA).
Receiving Stream — Barnard's Creek [seg. 18-80] is classified C; Swamp; it is not impaired [not 303d
listed, 2010]. Outfall 001 discharges to a UT draining southward for over —1.8 creek -miles of
swampland before reaching Barnard's Creek mainstem, ultimately draining another —1.4 creek -miles to
the Cape Fear River.
Parameters of Concern (POCs) - Reasonable Potential Analyses ("A) — Previous permit monitoring
requirements include Flow (limited as designed, 0.360 MGD), pH, WET testing, and the POC,
triehloroethylene (TCE). TCE has been limited at 92.4µg/L. However, the current Human Health
Stream Standard of 30µg/L TCE applies to this permit. This standard, together with the calculated
average flow rate (0.156 MGD — Jan2007-Feb2012) were used to establish reasonable potential using
the required Reasonable Potential Analyses (RPA). The TCE database shows reasonable potential by
calculating and comparing the Allowable 30µg/L vs. the MAXPredicted of 1,718µg/L (see attached
RPA). This requires the Division to lower the permit limit.

Given the revised Standard and zero flow stream conditions, the permit limit will be lowered from
92.4µ/L to 30µ/L TCE, allowing no credit for dilution. Moreover, the Division will increase the
monitoring frequency for TCE from the previous Quarterly to Monthly to better establish existing
effluent conditions. The limit will be footnoted to provide the Permittee option of petitioning to
reduce the TCE frequency, as data warrant, after sampling monthly for a minimum of one year (12
samples). pH sampling will also be increased from Quarterly to Monthly.
Monthly DMRs may not have included all samples analyzed by the Permittee during the previous
permit cycle (see analytical database dates listed as "Waver"). These sampling dates appear to
coincide with additional samples collected after a reported permit -limit exceedance. Considering the
absence of analytical data for chlorinated volatile organic compounds (VOCs) over the past 5-year
cycle, the Division will also reinstate an effluent spot-check; monitoring by EPA Method 601, to be
conducted annually.
The Division will admonish the Permittee to report M and all sampling and analytical events in
compliance with State REGs [NCAC 213.0506 (b) (3) (J)].

CORR Environmental Resources Inc. (CORR) as an authorized representative of AAF-
McQuay Inc (AMI) is submitting one original and two copies of the enclosed request for a
NPDES permit renewal for existing permit number NC0083658. The renewal application
.. is in reference to the groundwater remediation water treatment system at the former
Heatcraft property located at 602 Sunnyvale Drive in Wilmington, New Hanover County.
The following items are included with the renewal application:
• NPDES Permit Application — Short Form C-GW;
• Letter of delegation by AMI for CORR to act as an Authorized Representative to
prepare and submit the application;
• A topographic map indicating the location of the property and the outfall location;
• An Expanded Site Plan presenting prominent site features including the outfall
001 location along with longitude and latitude coordinates;
• A signed narrative description of the sludge management plan for the facility; and
• A data summary (as Table 1) of the 2010-2011 influent and effluent laboratory
_ testing results. Also included is the most recent aquatic toxicity testing report for
the quarterly discharge sampling conducted in May 2011.

AAF-McQuay Inc, a successor to the Refrigeration Products Group of SnyderGeneral
OR Corporation, was the former owner of the Heatcraft facility. The facility is located in New
Hanover County at 602 Sunnyvale Drive, Wilmington, North Carolina. SnyderGeneral
Corporation sold the Heatcraft facility in 1986 to Lennox Industries Inc. who then sold the
property to Port City Supply in late 2010. Lennox Industries continued to operate the
manufacturing facility until 1988 when the plant was shut down. In 1996, vacant space
in the plant building was leased to International Paper and ultimately to Port City Supply
who currently operates as S&R Packing and Crating. S&R provides shipping services to
local entities. Port City Supply purchased the property from Lennox in December 2010
and continues to operate the S&R Packing and Crating facility. S&R is not involved with
the groundwater remediation program at the property and does not utilize any hazardous
materials or generate any hazardous wastes.
Former manufacturing operations by the Refrigeration Products Group and Lennox at
the facility included the fabrication of heating and air conditioning equipment. Historically
the fabrication of air conditioning involved the stamping and machining of various metals
into refrigeration coils which required cleaning prior to testing and shipping. The
+� cleaning typically involved mechanical vapor degreasing using the organic solvent
trichloroethene (TCE). The use of TCE was discontinued and was replaced with 1,1,1
trichloroethane (TCA).
The facility recorded a release of TCA of approximately 400 to 500-gallons in 1983 on
the northwest side (rear) of the plant building. A second release of approximately 55-
�, gallons of TCA occurred in July 1987 also in the rear of the building. Subsequent to
these releases, the DEHNR — Division of Solid Waste Management required closure of
the second TCA release area by capping and implementation of a quarterly groundwater
monitoring program. In addition to the two TCA releases, the facility also experienced a
localized release of fuel oil from underground storage tanks on the southwest corner of
the property.
am SnyderGeneral Corporation began groundwater remediation from a single extraction well
at the site in 1994. Groundwater treatment was effected by air stripping technology to
remove the volatile organic compounds. During this initial phase of groundwater
MR remediation no attempt was made to address the localized fuel oil issue resulting from
the UST release.
In 2000 AMI authorized the removal and replacement of the existing air stripping
equipment with replacement with newer technology for greater efficiency. Concurrent
with that project, additional groundwater extraction wells were installed to better control
the contamination plume beneath the plant building along Sunnyvale Drive.

Sludge Management
Former Heatcraft Remediation Site

Currently no sludge or solid wastes are generated during the groundwater treatment
activities. No industrial waste water is included in the influent stream and all
groundwater contaminants are volatile organic compounds, primarily trichlrorethene,
trichloroethane and the constituent breakdown products. The contaminants are off -
gassed during the air stripping operation and no residuals remain after treatment. The
only solid waste generated is accumulated dirt and wind blown sand which collects in the
secondary containment area sump. These solids primarily include pine tree needles and
are removed as warranted to maintain operation of the sump. These solids are removed
from the sump and placed in a DOT approved 55-gallon drum for nearly immediate
disposal. The removal of sediments/solids from the sump happens on average every
two years or greater. The drum is properly disposed of as hazardous waste by contract
to an approved vendor to McQuay. Waste disposal transport manifest(s) and disposal
certification documents are filed on site when received from the vendor.
-� Future plans for the system may include the use of additional groundwater extraction
points at the rear of the facility former Heatcraft facility. If these additional extraction
points are required by the DNR Hazardous Waste Section, this remedial activity could
generate petroleum hydrocarbon based sludge or solids that are commingled with
dissolved volatile organic constituents. These materials if generated will be
containerized in DOT approved 55-gallon drums or could be stored in an appropriate
storage tank for scheduled removal by a permitted vacuum truck and disposed of as a
hazardous waste or recycled as appropriate or allowed by law.
Prior to any off -site shipment of sludge or solid materials, CORR will submit a Sludge
Management Plan containing a full analytical report, handling procedures and
disposal/recycling options for agency approval.

`�! 5, located parallel to Sunnyvale Drive manifold via double walled piping to a lift station.
The collected groundwater is then transferred from the lift station, via double walled
piping, over and across the roof of the plant building to the treatment system at the rear
of the plant building.

No significant modifications have been made to the groundwater treatment and
7 extraction system at the site since the 2006 NPDES permit renewal was issued. The air
stripping treatment process has remained the same. The only significant operations
issue has been the replacement of the lift station accumulation tank due to corrosion in
November 2010. The receiving water ditch on the south side of the property flows to the
east and Sunnyvale Drive where it flows via a culvert to the ditch parallel to Sunnyvale
Drive to the south where it flattens to a low lying area to the east toward Barnards Creek.
Barnards Creek is a local tributary to the Cape Fear River at a distance of several miles
from the outfall. The system contaminant load and source of the impacts remain as in
previous permit renewals.
