If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt of this letter. (2) CHRONIC TOXICITY TESTING REQUIREMENT The effluent discharge shall at no time exhibit observable inhibition of reproduction or significant mortality to Ceriodaphnia dubia at an effluent concentration of 90%. The permit holder shall perform at a minimum, quarterly monitoring using test procedures outlined in the "North Carolina Ceriodaphnia Chronic Effluent Bioassay Procedure," Revised February 1998. or subsequent versions or "North Carolina Phase II Chronic Whole Effluent Toxicity Test Procedure" (Revised -February 1998) or subsequent versions. The tests will be performed during the months of February, May, August, and November. Effluent sampling for this testing shall be performed at the NPDES permitted final effluent discharge below all treatment processes. If the test procedure performed as the first test of any single quarter results in a failure or ChV below the permit limit, then multiple -concentration testing shall be performed at a minimum, in each of the two following months as described in "North Carolina Phase II Chronic Whole Effluent Toxicity Test Procedure" (Revised -February 1998) or subsequent versions. The chronic value for multiple concentration tests will be determined using the geometric mean of the highest concentration having no detectable impairment of reproduction or survival and the lowest concentration that does have a detectable impairment of reproduction or survival. The definition of "detectable impairment," collection methods, exposure regimes, and further statistical methods are specified in the "North Carolina Phase II Chronic Whole Effluent Toxicity Test Procedure" (Revised -February 1998) or subsequent versions. All toxicity testing results required as part of this permit condition will be entered on the Effluent Discharge Monitoring Form (MR-1) for the months in which tests were performed, using the parameter code TGP3B for the pass/fail results and THP3B for the Chronic Value. Additionally. Total residual chlorine of the effluent toxicity sample must be measured and reported if chlorine is employed for disinfection of the waste stream. Should there be no discharge of flow from the facility during a month in which toxicity monitoring is required, the permittee will complete the information located at the top of the aquatic toxicity (AT) test form indicating the facility name, permit number, pipe number, county, and the month/year of the report with the notation of "No Flow" in the comment area of the form. The report shall be submitted to the Environmental Sciences Branch at the address cited above. Should the permittee fail to monitor during a month in which toxicity monitoring is required, monitoring will be required during the following month. Should any test data from this monitoring requirement or tests performed by the North Carolina Division of Water Quality indicate potential impacts to the receiving stream, this permit may be re -opened and modified to include alternate monitoring requirements or limits. NOTE: Failure to achieve test conditions as specified in the cited document, such as minimum control organism survival, minimum control organism reproduction, and appropriate environmental controls, shall constitute an invalid test and will require immediate follow-up testing to be completed no later than the last day of the month following the month of the initial monitoring. IWO% @ 5.0 MGD: 100, capped at 90 1.0 Proposed Changes Incorporated into Permit Renewal • Permit Expiration Date: The expiration date has been changed to December 31, 2006 in accordance with the Cape Fear River Basin permitting schedule. 2.0 Summary This is a permit renewal for a facility treating groundwater contaminated with halogenated solvents. Former manufacturing operations at the facility included the fabrication of heating and air conditioning equipment. The fabrication of air conditioning involved the stamping and machining of various metals into refrigeration coils which required cleaning prior to testing and shipping. The cleaning facility typically involved mechanical vapor degreasing using the organic solvent trichloroethene (TCE). The use of TCE was discontinued and replaced with 1, 1, 1 -trichloroethane (TCA). The facility recorded a release of TCA of approximately 400 to 500 gallons in 1983 in the northwest side (rear) of the plant. A second release of approximately 55 gallons of TCA occurred in July 1987. Subsequent to these releases, the Division of Solid Waste Management required closure of the second TCA release area by capping and implementation of a quarterly groundwater monitoring program. In addition to the two TCA releases, the facility also experienced a localized release of fuel oil from underground storage tanks on the southwest corner of the property. Groundwater remediation activities begun in 1994 by SnyderGeneral Corporation. Treatment included air stripping to remove VOCs. Newer equipment along with additional extraction wells was installed in 2000. The permit application notes that "significant" modifications to the treatment system have been made since the original NPDES permit was issued. The facility appears to be operating well. No permit violations were noted during the period of review. The last wasteload allocation was conducted in 1993. No industrial waste water is included in the influent stream and all groundwater contaminants are volatile organic compounds, primarily tdchlrorethene, tdchloroethane and the constituent breakdown products. The contaminants are off - gassed during the air stripping operation and no residuals remain after treatment. _ Future plans for the system may include the use of additional groundwater extraction points at the rear of the facility former Heatcraft facility. If these additional extraction points are required by the DNR Hazardous Waste Unit, this remedial activity could generate petroleum hydrocarbon based sludge or solids that are commingled with dissolved volatile organic constituents. These materials when and if generated will be containerized in DOT approved 55-gallon drums or could be stored in an appropriate storage tank for scheduled removal by a permitted vacuum truck and disposed of as a hazardous waste or recycled as appropriate or allowed by law. Prior to any off -site shipment of sludge or solid materials, CORR will submit a Sludge .. Management Plan containing a full analytical report, handling procedures and disposal/recycling options for agency approval. no WITH H1 ENVMONMENTAL RESOURCES, INC. o* Site History AAF-McQuay Inc, a successor to the Refrigeration Products Group of SnyderGeneral �. Corporation, was the former owner of the Heatcraft facility. The facility is located in New Hanover County at 602 Sunnyvale Drive, Wilmington, North Carolina. SnyderGeneral Corporation sold the Heatcraft facility in 1986 to Lennox Industries Inc. who currently owns the site. Lennox continued to operate the facility until 1988 when the plant was shut down. In 1996, vacant space in the plant building was leased to International Paper who currently operates a paper and cardboard recycling plant. Former manufacturing operations at the facility included the fabrication of heating and air conditioning equipment. Historically the fabrication of air conditioning involved the stamping and machining of various metals into refrigeration coils which required cleaning prior to testing and shipping. The cleaning typically involved mechanical vapor degreasing using the organic solvent trichloroethene (TCE). The use of TCE was discontinued and was replaced with 1,1,1 trichloroethane (TCA). The facility recorded a release of TCA of approximately 400 to 500-gallons in 1983 on the northwest side (rear) of the plant building. A second release of approximately 55- am gallons of TCA occurred in July 1987 also in the rear of the building. Subsequent to these releases, the DEHNR — Division of Solid Waste Management required closure of the second TCA release area by capping and implementation of a quarterly groundwater ,e monitoring program. In addition to the two TCA releases, the facility also experienced a localized release of fuel oil from underground storage tanks on the southwest corner of the property. SnyderGeneral Corporation began groundwater remediation from a single extraction wells at the site in 1994. Groundwater treatment was effected by air stripping technology to remove the volatile organic compounds. During this initial phase of groundwater remediation no attempt was made to address the localized fuel oil issue resulting from the UST release. In 2000 AMI authorized the removal and replacement of the existing air stripping equipment with replacement with newer technology for greater efficiency. Concurrent with that project, additional groundwater extraction wells were installed to better control the contaminate plume beneath the plant building along Sunnyvale Drive. The attached Figures identify the site as it currently exists with the addition of the new extraction wells and the air stripping units. CORR has attached the following Figures: Figure 1 — The USGS topographic map, 1:24,000 scale, Wilmington, North Carolina 1969, photo revised 1993. The map presents the approximate location of the single treatment system effluent outfall. Drawing 3 — The facility base map identifying all pertinent site features and landmarks including improvements and wells. Drawing 2 — The conceptual process flow diagram of the groundwater treatment system as currently configured. Extracted groundwater from EW-3 and recovery wells EW-2, EW-4 and EW-5, located parallel to Sunnyvale RM Drive manifold via double walled piping to a lift station. The collected groundwater is then transferred from the lift station, via double walled piping, over and across the roof of the plant building to the treatment system at the rear of the plant building. Drawing 12 — Depicts the layout of the treatment system as it is currently configured. Drawing 22 — Presents a detailed layout of the treatment system. Although significant modifications have been made to the groundwater treatment and extraction system at the site since the original NPDES permit was issued, the treatment process is essentially the same.
