Dear Mr. Powell:
Division personnel have reviewed and approved your application for renewal of the subject permit.
Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North
Carolina and the U.S. Environmental Protection Agency dated May 9,1994 (or as subsequently amended).
This final permit includes no major changes from the draft permit sent to you on June 20, 2007.
This permit includes a TRC limit that will take effect on March 1, 2009. If you wish to install dechlorination
equipment, the Division has promulgated a simplified approval process for such projects. Guidance for
approval of dechlorination projects may be viewed online at hM2://www.nccgl.net/news/ATCoverview.html.
In response to your comments on the draft permit, footnote 4 on the Effluent Limitations and Monitoring
Requirements page states you are required to monitor for total residual chlorine only if chlorine is used in the
treatment process (for example, if you backwash with finished, chlorinated water). For guidance on how to use
a composite sampler to sample an intermittent flow and where to locate a composite sampler, please refer to the
instrument's manual, or contact staff in the Wilmington Regional Office at 910-350-2004.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30) days
following receipt of this letter. This request must be in the form of a written petition, conforming to Chapter
150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings (6714 Mail
Service Center, Raleigh, North Carolina 27699-6714). Unless such demand is made, this decision shall be final
and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may
require modification or revocation and reissuance of the permit. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or permits
required by the Division of Land Resources, the Coastal Area Management Act or any other Federal or Local
governmental permit that may be required. If you have any questions concerning this permit, please contact

SUPPLEMENT TO PERMIT COVER SHEET
All previous NPDES Permits issued to this facility, whether for operation or
discharge are hereby revoked. As of this permit issuance, any previously issued
permit bearing this number is no longer effective. Therefore, the exclusive authority
to operate and discharge from this facility arises under the permit conditions,
requirements, terms, and provisions included herein.
Onslow Water and Sewer Authority is hereby authorized to:
1. Continue to discharge green sand filter and ion exchange backwash from a water
treatment facility.
The facility is located South of Dixon at the Dixon WTP at 6661 US Highway 17
in Onslow County.
2. Discharge from said treatment works at the location specified on the attached
map into an unnamed tributary to Stones Creek, classified SA HQW waters in
the White Oak River Basin.

Footnotes:
1. An exception to the composite sampling is provided by 15A NCAC 2B.0505 (C), which states that facilities
with design flows under 30,000 gallons per day may use grab samples to characterize their effluent.

3. The pH shall not be less than 6.8 standard units nor greater than 8.5 standard units.

A.' (2.) ACUTE TOXICITY PASS/FAIT. MONITORING (QRTRLY)
The permittee shall conduct acute toxicity tests on a cauartreriy basis using protocols defined in the
North Carolina Procedure Document entitled "Pass/Fail Methodology For Determining Acute Toxicity
In A Single Effluent Concentration" (Revised -July, 1992 or subsequent versions). The monitoring
shall be performed as a Mysidopsts bahia 24-how static test. The effluent concentration defined as
treatment two in the procedure document will be 90%. Effluent samples for self -monitoring purposes
must be obtained during representative effluent discharge below all waste treatment. The tests will be
performed during the months of January, April, July and October.
All toxicity testing results required as part of this permit condition will be entered on the Effluent
Discharge Monitoring Form (MR-1) for the month in which it was performed, using the parameter
code TGE3E. Additionally, DWQ Form AT-2 (original) is to be sent to the following address:

Completed Aquatic Toxicity Test Forms shall be filed with the Environmental Sciences Section no
later than 30 days after the end of the reporting period for which the report is made.
Test data shall be complete and accurate and include all supporting chemical/physical
measurements performed in association with the toxicity tests, as well as all dose/response data.
Total residual chlorine of the effluent toxicity sample must be measured and reported if chlorine is
employed for disinfection of the waste stream.
Should there be no discharge of flow from the facility during a month in which toxicity monitoring is
required, the permittee will complete the information located at the top of the aquatic toxicity (AT)
test form indicating the facility name, permit number, pipe number, county, and the month/year of
the report with the notation of "No Flow" in the comment area of the form. The report shall be
submitted to the Environmental Sciences Section at the address cited above.
Should the permittee fail to monitor during a month in which toxicity monitoring is required, then
monthly monitoring will begin immediately until such time that a valid test is submitted. Upon
submission of a valid test, this monthly test requirement will revert to quarterly in the months
specified above.
Should any test data from this monitoring requirement or tests performed by the North Carolina
Division of Water Quality indicate potential impacts to the receiving stream, this permit may be re-
opened and modified to include alternate monitoring requirements or limits.
NOTE: Failure to achieve test conditions as specified in the cited document, such as minimum
control organism survival and appropriate environmental controls, shall constitute an invalid test
and will require immediate follow-up testing to be completed no later than the last day of the month
following the month of the initial monitoring.

We have reviewed the referenced draft permit for the Dixon Water Treatment Plant
transmitted by your letter dated June 20, 2007. We have the following questions for your
consideration:
1. Since we do not disinfect our effluent with chlorine prior to its discharge, why is it
proposed that we monitor total residual chlorine?
2. Due to the nature of the operation, the flow from the lagoons is intermittent. How do
we use the composite sampler to sample an intermittent flow?
3. As you are aware, the receiving stream is tidal resulting in a submerged outfall.
Where do we need to locate the proposed composite sampler?

PART II - BACKGROUND AND HISTORY
Facility had two 0.915 million gallon settling lagoons (with 60 ml HDPE liners) in series.
PART III - DESCRIPTION OF DISCHARGE AND TREATMENT WORKS

Facility recently constructed another settling lagoon with 915,000 gallon capacity lined with 60
ml HDPE. Total treatment lagoons are three 915,000 gallon lined lagoons.
4) Date(s) and construction activities allowed by previous Authorizations to Construct issued in the
previous two ,years:
A to C given on July 25, 2006 from CG&L for the construction of one new 915,000 gallon
settling lagoon.
5) Please provide a description of existing or substantially constructed wastewater treatment
acilities:
This water treatment plant utilizes conventional treatment (suspended solids settling), green sand
filtration for iron and manganese removal, ion exchange for calcium and magnesium removal,
carbon filtration for TOC removal. The facility backwashes with chemically treated potable
water. Potential treatment additives are potassium permanganate and chlorine for disinfection,
and fluoridation. Treated water is used for backwashing.
The facility has three lined lagoons with a 915,000 gallon capacity each. Influent introduced to
the first lagoon passes through all three lagoons in series before exiting to the effluent well.
Influent introduced to the third lagoon makes a one pass across the third lagoon to the effluent
well. Settling time is considerably shorter. Typically influent would be introduced to the first
lagoon to provide adequate detention time for settling. The influent flow is 1300 gpm as a batch
feed. There are no chemical additions to promote settling. Effluent from the effluent vault is
pumped using one 50 gpm pump (driven by one 3 horsepower motor) and a 500 gpm pump
(driven by one 20 horsepower pump).

any recoverable metals from the treatment of groundwater (should potential contamination
exist).

The facility dewaters the solids from the lagoon in order to pass the paint filter test for
disposal in the Onslow County Landfill. The schedule for removing solids is every three
years to maintain adequate treatment capacity for settling.
10) Compliance history for this facility within the past permit cycle is as follows:
Excellent, no history of compliance problems within the last permit cycle with the exception
of failure to secure an A to C to add the additional lagoon for treatment.

PART IV - OTHER PERTINENT INFORMATION

Per 15A NCAC 02B.0224 (ii) Total suspended solids: Discharges of total suspended solids
(TSS) shall be limited to effluent concentrations of 10 mg/L for trout waters and PNAs.
Per 15A NCAC 02B.0244 (vii) Toxic substances: In cases where complex wastes (those
containing or potentially containing toxicants) may be present in a discharge, a safety factor shall
be applied to any chemical or whole effluent toxicity allocation. The limit for a specific
chemical constituent shall be allocated at one-half of the normal standard at design conditions.
Whole effluent toxicity shall be allocated to protect for chronic toxicity at an effluent
concentration equal to twice that which is acceptable under design conditions. In all instances
there may be no acute toxicity in an effluent concentration of 90 percent.

PART V - EVALUATION AND RECOMMENDATIONS
The pen -nit limitation for TSS should be 10 mg/L due to the discharge to PNAs (HQW). Please see
comments in Part IV. 2.Other Pertinent Information, Special Monitoring or Limitations (including
toxicity requests) above. Please include the total residual chlorine limit of 17 µg/L. Pursuant to
Permitting Strategy for Ion Exchange WTPs issued in January 2004, include the minimum
monitoring requirements as per the attachment (Appendix B page 5). The monitoring frequency is
based upon the flow, therefore since this facility discharges less than 0.5 MGD, the monitoring

Fluoride limit of 1800 µg/L should be imposed and manganese should be monitored (since it is not a
parameter of concern for IE WTPs) per the Permitting Strategy for Greensand Filtration Water
Treatment Plant issued January 2004.
The Wilmington Regional Office recommends reissuance of the permit in accordance with the White
Oak River Basin Plan permitting strategy provided no significant adverse public comment is
received. The Wilmington Regional Office also requests that the recommendations provided herein
are considered in the reissuance of Permit No. NC0058271.
