1. Gary Hudson of the Winston-Salem Regional Office of the NC Division of Water Quality (DWQ) conducted
compliance evaluation inspections (CEI) at the subject facility on the subject permits on September 27,
2011. Your assistance and cooperation during the inspection was greatly appreciated. Inspection
checklists are attached for your records and inspection findings are summarized below.
2. The facility is located at 815 Buxton Street, in Winston-Salem, Forsyth County, North Carolina. Stormwater
related to industrial activity is discharged to Peters Creek, which is currently classified as Class C waters in
the Yadkin Pee Dee River basin.

Documentation & Monitoring Review
1. Permit
Hanes Dye & Finishing holds Certificate of Coverage # NCG170009 for the discharge of
stormwater from the facility. The General Permit became effective August 1, 2009 and expires
July 31, 2014.
2. Records/Reports
Part II, Section A of the permit requires the development of a Stormwater Pollution Prevention
Plan. A copy of the SPPP for Hanes Dye & Finishing was available for review during the
inspection. The plan appeared thorough and complete.
3. Facility Site Review
The site appeared clean and well managed. No concerns or problems were noted.

Stormwater leaves the Hanes Dye & Finishing property reaching Peters Creek, Class "C waters
in the Yadkin Pee Dee River Basin.
5. Monitoring Program
Part II, Section B of the permit requires semi-annual qualitative (visual) monitoring to be
performed and documented once in the spring (April- June) and once in the fall (September —
November). A records review during the inspection showed that Hanes Dye & Finishing had
performed and documented the required qualitative monitoring in 20010 and 2011. Please note
that you are not required to submit these forms to the Division, however, the records must be
kept on file at the facility for a period of five years.
The analytical monitoring listed in Section C of the permit for on -site vehicle maintenance
operations is not required since Hanes Dye & Finishing does not use more than 55 gallons of new
motor oil per month when averaged over the calendar year.

In accordance with your renewal application [received on February 2, 20071, the Division is renewing
Certificate of Coverage (CoC) NCG500352 to discharge under NCG500000. This CoC is issued pursuant to the
requirements of North Carolina General Statue 143-215.1 and the Memorandum of Agreement between North
Carolina and the US Environmental Protection agency dated May 9, 1994 [or as subsequently amended].
If any parts, measurement frequencies or sampling requirements contained in this General Permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual permit
application. Unless such demand is made, the certificate of coverage shall be final and binding.
Please take notice that this Certificate of Coverage is not transferable except after notice to the
Division. The Division may require modification or revocation and reissuance of the certificate of coverage.
Contact the Winston-Salem Regional Office prior to any sale or transfer of the permitted facility.
Regional Office staff will assist you in documenting the transfer of this CoC.
This permit does not affect the legal requirements to obtain other permits which may be required by
the Division of Water Quality or permits required by the Division of Land Resources, Coastal Area
Management Act or any other Federal or Local governmental permit that may be required.

➢ Site map. If the discharge is not directly to a stream, the pathway to the receiving stream must
be clearly indicated. This includes tracing the pathway of a storm sewer to its discharge point.
➢ Authorization for representatives. If this application will be submitted by a consulting
engineer (or engineering firm), include documentation from the Permittee showing that the
consultant submitting the application has been designated an Authorized Representative of the
applicant.

falsifies, tampers with or knowingly renders inaccurate any recording or monitoring device or method required to be operated or maintained under
Article 21 or regulations of the Environmental Management Commission implementing that Article, shall be guilty of a misdemeanor punishable by
a fine not to exceed $25,000, or by imprisonment not to exceed six months, or by both. (18 U.S.C. Section 1001 provides a punishment by a fine

In response to your inquiry about the low -flow characteristics (7Q10) for your locations on Peters Creek in
Winston-Salem (Forsyth County), the following information is provided:
A check of the low -flow files here at the USGS North Carolina Water Science Center does not indicate a previous
determination of low -flow characteristics with values identical to those specified in your email. The estimates are
based on statistical relations applicable for streams in the Winston-Salem area provided in a USGS statewide
low -flow report published in 1993. This statewide report is USGS Water -Supply Paper 2403, 'Low -flow
characteristics of streams in North Carolina" (Giese and Mason, 1993). The information and data provided in this
report represents the "most recent' low -flow characteristics for streams in this area.
An online version of the report is available through http://pubs.er.usgs.gov/usgspubstwsp/wsp2403. The report
provides the low -flow characteristics (based on data through 1988) via regional relations and at -site values for
sites with drainage basins between 1 and 400 sgmi and not considered or known to be affected by regulation
and/or diversions.
If you access the report, please note the online report files are provided in the ".DJVU" format and require a
particular Lizardtech plug-in, also available through a link displayed on the page. Or you can click an adjacent
link that will allow you to view the report as a group of images without the need for a plug-in.
Low -flow characteristics for ungaged locations are determined by assessing a range of low -flow yields (expressed
as flow per sqmi drainage area, or cfsm) at nearby locations where estimates have been previously determined.
Based on the information gleaned from your email, it appears that a drainage area of 2 sgmi was used to
determine the previous values (cited in your email). Applying the applicable 7010 statistical relation in WSP 2403
to this drainage area results in a 7Q10 discharge (0.28 cfs, rounded to 0.3 cfs) with a low -flow yield of about 0.14
cfsm. Please be aware that use of the statistical relation applicable to Peters Creek produces varying low -flow
yields with changing drainage areas. For instance, at a drainage area of 4 sgmi, the relation estimates a 7Q10
discharge with a yield equivalent to about 0.1 cfsm.
In July 2000, an analysis for another low -flow request at Peters Creek at Winston-Salem (station id 02115845,
drainage area 5.18 sgmi) was completed, and the resulting 7Q10 low -flow yield based on assessment of nearby
sites was about 0.13 cfsm, almost identical to the yield cited above from the WSP 2403 equation.
However, it should be pointed out that this low -flow estimate is reflective of natural -flow type conditions. Because
your basin is affected by development, it raises the question as to how low flows are affected. In the past, there
has been discussion within the hydrologic community that increased development eventually reduces the low
flows because increased impervious areas reduce the base flows used to sustain low flow during drought periods.
Putting all of the above information together, it appears appropriate to use a low -flow yield range of 0.1 to 0.14
cfsm, with the estimated 7Q10 discharge ranging from 0.2 to 0.3 cfs. And given the uncertainty associated with
how development has affected flows in this basin, it may be prudent to use a lower value of 0.2 cfs for 7Q10

discharge pending further data collection and/or analyses.
As for the other flow characteristics you cited in your email, these values likewise are based on information from
WSP 2403 and should be considered okay, again pending further data collection and/or analyses.
