Johnston County
On September 24, 2015, from the Raleigh Regional Office conducted an inspection of the subject
facilities. The cooperation of Jeff Duffield, Senior Maintenance Technician, was appreciated. Findings
during the inspection were as follows:
1. The current Certificate of Coverage permit became effective August 13, 2012.
2. The noncontact cooling water discharged from this system is comprised of water used to cool air
compressors. Water used to cool the tubing from extruders is discharged to the sanitary sewer.
3. The permit requires semiannual monitoring of flow, temperature, total residual chlorine, oil and
grease, pH, and chemical oxygen demand (COD). As discussed, COD is not required to be
monitored if no corrosion inhibitors, oxygen scavengers, biocides, or cleaning solvents are
added to the system (footnote 4 of the permit).
4. Downstream monitoring is performed in a ditch adjacent to the railroad tracks.
5. All monitoring appears to be conducted in accordance with the permit. Microbac collects and
analyzes samples.
6. No problems were noted at the facility.

In accordance with your renewal application [received on January 17, 20071, the Division is renewing
Certificate of Coverage (CoC) NCG500104 to discharge under NCG500000. This CoC is issued pursuant to the
requirements of North Carolina General Statue 143-215.1 and the Memorandum of Agreement between North
Carolina and the US Environmental Protection agency dated May 9, 1994 [or as subsequently amended].
If any parts, measurement frequencies or sampling requirements contained in this General Permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual permit
application. Unless such demand is made, the certificate of coverage shall be final and binding.
Please take notice that this Certificate of Coverage is not transferable except after notice to the
Division. The Division may require modification or revocation and reissuance of the certificate of coverage.
Contact the Raleigh Regional Office prior to any sale or transfer of the permitted facility.
Regional Office staff will assist you in documenting the transfer of this CoC.
This permit does not affect the legal requirements to obtain other permits which may be required by
the Division of Water Quality or permits required by the Division of Land Resources, Coastal Area
Management Act or any other Federal or Local governmental permit that may be required.

➢ Site map. If the discharge is not directly to a stream, the pathway to the receiving stream must
be clearly indicated. This includes tracing the pathway of a storm sewer to its discharge point.
➢ Authorization for representatives. If this application will be submitted by a consulting
engineer (or engineering firm), include documentation from the Permittee showing that the
consultant submitting the application has been designated an Authorized Representative of the
applicant.

The subject permit expires on July 31,1997. North Carolina General Statute 143.215.1(c) requires that an application for
permit renewal be filed in a timely manner. If continuation of the permit is desired, please submit the following
information within 30 days of receipt of this letter:
1. A letter requesting the renewal.
2. A list of all chemical additives (if any) in use or proposed for use at your facility (biocides, corrosion inhibitors, etc.).
3. The completed invoice form (copy attached).
4. A processing fee of $400. The check should be made payable to the North Carolina Department of Environment,
Health, and Natural Resources which may be abbreviated as NCDEHNR.
Failure to request renewal within this time period may result in a civil assessment of at least $250.00. Larger penalties
may be assessed depending upon the delinquency of the request.
If operation of a discharge or waste treatment facility is to occur after the expiration date of the permit, or if
continuation of the permit is desired, it must not be allowed to expire. Operation of the waste treatment works or
continuation of a discharge after the expiration date would constitute a violation of NCGS 143-215.1 and could result in
assessment of civil penalties of up to $10,000 per day.

In accordance with your application for discharge permit received on February 17, 1993, we are
forwarding herewith the subject certificate of coverage to discharge under the subject state - NPDES general
permit. Issuance of this certificate of coverage supercedes the individual NPDES permit No. NCO080551.
This permit is issued pursuant to the requirements of North Carolina General Statute 143-215 .1 and the
Memorandum of Agreement between North Carolina and the US Environmental Protection agency dated
December 6, 1983.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual permit
application. Unless such demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the Division
of Environmental Management. The Division of Environmental Management may require modification or
revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by the
Division of Environmental Management or permits required by the Division of Land Resources, Coastal Area
Management Act or any other Federal or Local governmental permit that may be required.
