King is hereby authorized to discharge wastewater from a facility located at Countryside Manor 7700 US Hwy 158 Stokesdale Guilford County to receiving waters designated as Troublesome Creek in the Cape Fear River Basin in accordance with effluent limitations, monitoring requirements, and other conditions set forth in Parts I, II, III, and IV hereof.
