Division personnel have reviewed and approved your application for renewal of the subject permit.
Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued pursuant
to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between North Carolina and the U.S. Environmental Protection Agency dated October 15, 2007 (or as
subsequently amended).
This final permit includes one significant change to its terms from those found in the draft permit sent to
you on March 27, 2013. This change is:
a Reporting of Total Kjeldahl Nitrogen and Nitrites + Nitrates concentrations have been removed from
the permit.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty
(3 0) days following receipt of this letter. This request must be in the form of a written petition,
conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of
Administrative Hearings (6714 Mail Service Center, Raleigh, North Carolina 27699-6714). Unless
such demand is made, this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may
require modification or revocation and reissuance of the permit. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or permits
required by the Division of Land Resources, the Coastal Area Management Act or any other Federal or
Local governmental permit that may be required.

In compliance with the provisions of North Carolina General Statute 143-215.1, other lawful standards

to receiving waters designated as an unnamed tributary to Middle Creek in the Neuse River Basin in
accordance with effluent limitations, monitoring requirements, and other conditions set forth in Parts I,
II, III and IV hereof.
The permit shall become effective July 1, 2013.
This permit and the authorization to discharge shall expire at midnight on April 30, 2018.

SUPPLEMENT TO PERMIT COVER SHEET
All previous NPDES Permits issued to this facility, whether for operation or discharge are hereby
revoked. As of this permit issuance, any previously issued permit bearing this number is no longer
effective. Therefore, the exclusive authority to operate and discharge from this facility arises under the
permit conditions, requirements, terms, and provisions included herein.
Aqua North Carolina, Inc.
is hereby authorized to:
1. Continue to discharge wastewater from a Greensand filter water treatment plant, with additional
wastewater treatment provided by a tablet dechlorinator and a final sand bed filter prior to discharge.
This facility is located at the Oak Hollow Subdivision Water Treatment Plant, off NC Highway 42,
near Mount Pleasant, in Wake County.
2. Discharge at the location specified on the attached map into anunnamed tributary to Middle Creek,

Hi Shari,
Thank you for the WRC's comments on this draft permit. I just wanted to pass along a few bits of information. This
water treatment plant discharge is relatively small. I looked at data reported for the facility for 2011— 2012. Reported
flow ranged from 1,250 to 3,900 gallons per day. The effluent does receive dechlorination and passes through a sand
filter before it is discharged to the stream. During that same period of time, chlorine values ranged from <12 µg/L to 33
µg/L. Most reported values were at or near the low end of that range. Our records did not show any noncompliance
with permit limits during that time (due to questionable reliability of reported chlorine values that are near the method
detection levels, DWQ considers reported values that are <50 µg/L to be compliant with TRC limits).
I hope this may satisfy some of your concerns about the discharge. Unless we receive further comment from you or
other interested parties, it's likely that the permit will be issued as drafted.

Biologists with the North Carolina Wildlife Resources Commission have reviewed the subject
document. Our comments are provided in accordance with provisions of the Clean Water Act of 1977 (as
amended), Fish and Wildlife Coordination Act (48 Stat. 401, as amended; 16 U.S.C. 661-667e), North
Carolina General Statutes (G.S. 113-131 et seq.), and North Carolina Administrative Code 15A NCAC
10I.0102.
Aqua North Carolina, Inc. has applied for a renewal of their NPDES permit to discharge treated
filter backwash wastewater to an unnamed tributary to the Middle Creek in the Neuse River Basin.
According to the public notice, Total Residual Chlorine is currently water quality limited.
There are records for the federal and state endangered dwarf wedgemussel (Alasmidonta
heterodon); the federal species of concern and state endangered Atlantic pigtoe (Fusconaia masoni); the
federal species of concern and state significantly rare pinewoods shiner (Lythrurus matutinus); the state
threatened creeper (Strophitus undulatus), Roanoke slabshell (Elliptio roanokensis), triangle floater
(Alasmidonta unduaata), Eastern lampmussel (Lampsilis radiata), Carolina fatmucket (Lampsilis radiata
conspicua), and least brook lamprey (Lampetra aepyptera); and the state special concern North Carolina
spiny crayfish (Orconectes [Procericambarus] carolinensis) in Middle Creek. Also the Significant
Natural Heritage Area — Middle Creek (Wake/Johnston) Aquatic Habitat — is located in this section of the
Middle Creek.
Chlorine is acutely toxic and can form secondary compounds that are detrimental to aquatic life.
Valenti (2006) concluded the impact of long term exposure to low doses of chlorine may impact juvenile
mussels and reduce the chance of them being recruited to the reproducing population. Due to the
presence of several rare and sensitive species in Middle Creek, we offer the following recommendations
to minimize impacts to aquatic resources.

SLUDGE MANAGEMENT PLAN

No sludge will be treated on any wastewater treatment plant site operated
by Aqua N.C., Central Division. '
Where practical, sludge removed from a Aqua N.C., Central Division
facility will be transported via a contract hauler tQ another. Aqua N.C.,
Central Division facility for the purpose of 'seeding" a new or under loaded
plant
Unusable (or "dead") sludge will. be removed by a contract hauler and
properly disposed of in accordance with NCGS.1 4�3-215.1. Contract
haulers used by Aqua T.C., Central Division will be required to report the
quantity of sludge transported and identify the location of the proposed
disposal site if the sludge is not taken to an existing plant operated by Aqua
N.C., Central Division. Aqua N.C., Central Division has not entered into
any agreement to accept sludge into its facilities from plants -not owned by
them
Aqua N.C., Central Division will keep records on the quantity of sludge
removed from each facility, the name of the contract hauler, and the
destination of the sludge (whether used in another plant or disposed o fl.
