Diamond Head Development

A letter of request for an Authorization to Construct was received December
27, 1988, by the Division and final plans and specifications for the subject project
have been reviewed and found to be satisfactory. Authorization is hereby granted
for the construction of a 0.10 MGD extended aeration wastewater treatment plant
consisting of a 20,000 gallon equalization tank with dual 53 GPM submersible pump
and high water alarm, a bar screen, a totalizer flow meter, two (2) 50,000 gallon
aeration tanks, a 16,667 gallon clarifier, a 10,000 gallon sludge holding tank,
three (3) 300 cfm blowers; a tertiary treatment unit with 70 square feet of
treatment area, a 25 cfm blower, clean water tank and mudwell, a 2,090 gallon
chlorine contact tank with two (2) tablet chlorinators, a 832 gallon post aeration
tank with an effluent pump, a stand-by generated and all associated valves, pipes
and appurtenances to serve Diamond Head Development in Iredell County.
This Authorization to Construct is issued in accordance with Part III paragraph
B of the NPDES Permit No. NG0074772 issued December 16, 1988, and shall be
subject to revocation unless the wastewater treatment facilities are constructed in
accordance with the conditions and limitations specified in Permit No. NC0074772.
The Permittee must employ a certified wastewater operator in accordance with
Part III paragraph C of the referenced permit.
The sludge generated from these treatment facilities must be disposed of in
accordance with G.S. 143-215.1 and in a manner approvable by the North Carolina
Division of Environmental Management.
The Mooresville Regional Office, telephone nuinber'704-663-1699 shall be
notified at least twenty-four (24) hours in advance of operation of these
facilities. Such notification to the Regional Supervisor shall be made during the
normal office hour from 8:00 a.m. until 5:00 p.m. on Monday through Friday,
excluding State Holidays.

In event the facilities fail to perform satisfactorily in meeting its NPDES
permit effluent limits, the permittee shall take such immediate corrective action 6s .
may be required by this Division, including the construction of additional
wastewater treatment and disposal facilities.
Upon completion of construction and prior to operation of this permitted
facility, a certification must be received from a professional engineer certifying:.
that the permitted facility has been installed in accordance with the approved plans
and specifications. Mail the Certification to the Permits and Engineering Unit,
P.O. Box 27687, Raleigh, N.C. 27611-7687.
The Operational Agreement between the Permittee and the Environmental
Management Commission is incorporated herein by reference and is a condition of theis Permit. Noncompliance with the terms of the Operational Agreement shall subject;`
the Permittee to all sanctions provided by G. S. 143-215.6 for violation of or
failure to act in accordance with the terms and conditions of this Permit.
A copy of the approved plans and specifications shall be maintained on file
the permittee for the life of the project.
