Dear Mr. Summey:
A letter of request for an Authorization to Constr-.ict was received July 25,
19883 by the Division and final plans and specifications -for the subject project
have been reviewed and found to be satisfactory. Authorization is hereby granted
for the construction of water treatment plant sludge settling ponds consisting of
two lagoons (2.0 MG total capacity) lined with a 40 millimeters thick impervious
liner with discharge of decanted effluent- into an unnam(.;d tributary to Second Broad
River.
This Authorization to Construct is issued in accordance with Part III paragraph
B of the NPDES Permit No. NCO074306 issued July 25, 1988, and shall be subject to
revocation unless the wastewater treatment facilities are constructed in accordance
with the conditions and limitations specified in Permit No. NC0074306.
The Permittee must employ a certified wastewater operator in accordance with
Part III paragraph C of the referenced permit.
The sludge generated from these treatment facilities must be disposed of in
accordance with G.S. 143-215.1 and in a manner approvable by the North Carolina
Division of Environmental Management.
The Asheville Regional Office, telephone number 704/251-6208 shall be
notified at least twenty-four (24) hours in advance of operation of these
facilities. Such notification to the Regional Supervisor shall be made during the
normal office hour from 8:00 a.m. until 5:00 p.m. on Monday through Friday,
excluding State Holidays.

In event the facilities fail to perform satisfactorily in meeting its NPDES
permit effluent limits, the permittee shall take such immediate corrective action as
may be required by this Division, including the construction of additional
wastewater treatment and disposal facilities.
Upon completion of construction and prior to operation of this permitted
facility, a certification must be received from a professional engineer certifying
that the permitted facility has been installed in accordance with the approved plans
and specifications. Mail the Certification to the. Permits and Engineering Unit,
P.O. Box 27687, Raleigh, N.C. 27611-7687.
A set of approved plans and specifications for the subject project must be
retained by the applicant for the life of the NPDES Permit. -
Diversion or bypassing of the untreated wastewater from the settling lagoons is
prohibited.
Freeboard in the lagoons shall not be less than two feet.
The interior surfaces of the two alum sludge storage lagoons shall be completed
lined with a 40-mil thick HDPE liner.
Following installation and inspection of the lagoon liners, and prior to waste
disposal operations, certification of the liners compliance with approved
construction specifications and the liners integrity shall be provided to the
Department by the project engineer.
Any groundwater quality monitoring, as deemed necessary by the Department,
shall be provided.
