Dear Mr. Fennell:
Division personnel have reviewed and approved your application for renewal of the subject permit.
Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North
Carolina and the U.S. Environmental Protection Agency dated May 9, 1994 (or as subsequently amended).
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to
you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt of
this letter. This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina
General Statutes, and filed with the Office of Administrative Hearings (6714 Mail Service Center, Raleigh, North
Carolina 27699-6714). Unless such demand is made, this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may require
modification or revocation and reissuance of the permit. This permit does not affect the legal requirements to obtain
other permits which may be required by the Division of Water Quality or permits required by the Division of Land
Resources, the Coastal Area Management Act or any other Federal or Local governmental permit that may be
required.

TO DISCHARGE WASTEWATER UNDER THE
NATIONAL POLLUTANT DISCHARGE ELIMINATION SYSTEM
In compliance with the provision of North Carolina General Statute 143-215.1, other lawful
standards and regulations promulgated and adopted by the North Carolina'-"O'nvironmental
Management Commission, and the Federal Water Pollution Control Act, as amended,

is hereby authorized to discharge wastewater from a facility located at the
Pleasant Cove Rest Home
189 Robinson Cove Road
Candler
Buncombe County
to receiving waters designated as Pole Creek in the French Broad River Basin
in accordance with effluent limitations, monitoring requirements, and other
conditions set forth in Parts I, II, III and IV hereof.
This permit shall become effective March 1, 2001.
This permit and authorization to discharge shall expire at midnight on October 31, 2005.

SUPPLEMENT TO PERMIT COVER SHEET
Havon, Inc., is hereby authorized to:
1. Continue to operate an existing 0.012 MGD extended aeration wastewater
treatment plant with the following components:
■ Aeration basin
■ Clarifier
■ Chlorine contact chamber with tablet chlorination
■ Lift station and forcemain outfall
This facility is located at the Pleasant Cove Rest Home on Robinson Cove Road
near Candler in Buncombe County.
2. After receiving an Authorization to Construct from the Division, modify the
treatment system to increase the total plant design capacity to 0.025 MGD.
3. Discharge from said treatment works at the location specified on the attached
map into Pole Creek, classified C waters in the French Broad River Basin.

If .this facility is built in phases, plans and specifications for the next phase to be constructed must
be ' submitted when the flow to the existing equipment reaches 80% (0.0096 MGD) of the existing
permitted flow (0.012 MGD). At no time may the flow tributary to the treatment system exceed the
design capacity of.the existing equipment.

Dear Mr. Fennell:
Enclosed with this letter is a copy of the draft permit for the subject facility. Please review the draft very
carefully to ensure thorough understanding of the conditions and requirements it contains.
There have been no substantial changes from the existing permit. However, please clarify whether the
installed treatment system has a design capacity of 0.012 MGD (the current permitted capacity) or
0.025 MGD (the capacity you listed on your renewal application. I can be reached at the telephong
number or address at the bottom of this page.
Submit any comments tome no later than thirty days following your receipt of the draft Comments
should be sent to the address listed at the bottom of this page. If no adverse comments are received from
the public or from you, this permit will likely be issued in December, with an effective date of January 1,
2001.
