Dear Mr. Weber:
A letter of request for an Authorization to Construct was received February
22, 1989, by the Division and final plans and specifications for the subject project
have been reviewed and found to be satisfactory. Authorization is hereby granted
for the construction of a 0.015 MGD wastewater treatment facility consisting of a
bar screen, a 3000 gallon equalization tank, two (2) 7,500 gallon aeration tanks,
two (2) 2,500 gallon clarifiers, a 2,333 gallon sludge holding tank, three (3) 95
CFM blowers, two (2) dual media 12 square foot filters with a 1,975 gallon backwash
tank, a 775 gallon chlorine contact tank, a tablet chlorinator, a 675 gallon post
aeration tank, standby generator, and approximately 1000 feet of discharge line to
the discharge point permitted in NC0075205.
This Authorization to Construct is issued in accordance with Part III paragraph
B of the NPDES Permit No. NC0075205 issued April 5, 1989, and shall be subject to
revocation unless the wastewater treatment facilities are constructed in accordance
with the conditions and limitations specified in Permit No. NC0075205.
Upon 50% completion of the subject facilities the Permittee must employ a
certified wastewater operator in accordance with Part III paragraph C of the
referenced permit.
The sludge generated from these treatment facilities must be disposed of in
accordance with G.S. 143-215.1 and in a manner approvable by the North Carolina
Division of Environmental Management.
The Mooresville Regional Office, telephone number 704/663-1699 shall be
notified at least twenty-four (24) hours in advance of operation of these
facilities. Such notification to the Regional Supervisor shall be made during the
normal office hour from 8:00 a.m. until 5:00 p.m. on Monday through Friday,
excluding State Holidays.

In event the facilities fail to perform satisfactorily in meeting its NPDES
permit effluent limits, the permittee shall take such immediate corrective action as
may be required by this Division, including the construction of additional
wastewater treatment and disposal facilities.
The sand media of the tertiary filters must comply with the Division's sand
specifications. The engineer's certification will be evidence that this
certification has been met.
Upon completion of construction and prior to operation of this permitted
facility, a certification must be received from a professional engineer certifying
that the permitted facility has been installed in accordance with the approved plans
and specifications.
A set of approved plans and specifications for the subject project must
be retained by the applicant for the life of the project.
