Certification Statement: I Michael Wagner certify to the best of my knowledge that the addition of the volume of wastewater to be permitted in this project has been evaluated along the route to the receiving wastewater treatment facility and that the flow from this project is not anticipated to cause any capacity related sanitary sewer overflows or overburden any downstream pump station en route to the receiving treatment plant under normal circumstances, given the implementation of the planned improvements identified in the planning assessment where applicable. This analysis has been performed in accordance with local established policies and procedures using the best available data. The primary reason for the requested permit modification is due to a request by the Town of Fuquay-Varina to utilize an alternate force main route to the Terrible Creek WWTP. The objective of the Town's request is to bypass the existing Basal Creek Sewer Pump Station. The quantity of proposed force main and discharge location remains unchanged, but an alternate segment of an abandoned force main will be returned to service and utilized to convey flow to the Terrible Creek WWTP. The proposed sewer quantities stated in the application have remained unchanged from the approved permit.This project will consist of 1,127 LF of 8" DIP and 1,294 LF of 8"C-900, and 3,080 LF of SDR-35 gravity sewer as well as 2,134 LF of 6" C-900 sewer force main located along Bass Lake Road and James Slaughter Road in Fuquay- Varina. The approved permit proposed sewer service for 145 single family homes and the flow values have remained unchanged. The proposed project will meet all of DWR's minimum design criteria for sanitary sewer extensions as well as DWR's minimum design criteria for pump stations/force mains. The design flow rate for this project remains at 38,100 gpd based on NCAC 2T. Is this an ARPA funded project? If so, please include the DWI assigned ARPA project number in your response. 2. Section III only states Bryant Inge as the consultant for this project but the narrative states there are two. Please provide David McClure's contact information since it is not provided in the FTA. a. According to the narrative, the FTA includes separate signature pages for the GS and PS/FM. I only have one signature page from David McClure for the PS criteria. Please provide the signature page for the GS. 3. According to the narrative, the flow rate for the project "remains 25,938 GPD", but according to the existing permit the flow is 38,100 GPD. Please clarify and provide an updated Page 2 of the FTA or narrative. a. Also, please update Section V1.7. of the FTA to reflect the correct flow of wastewater generated by the project. i. Section V1.8. is filled out correctly since this modification does not change the wastewater generated by the project. 4. According to our records,the downstream sewer is WQ0040523 Basal Creek Townhomes Phases 1-8, please confirm that this is no longer going to be the downstream sewer connection due to relocating the GS and PS/FM. 5. According to Section IX.S. of the FTA, 401/404 authorization is needed for the project. Please provide the Certification/ID Number and status for the 401/404 authorization. This information can also be included in the narrative. 6. The topographical and aerial map submitted are not sufficient. According to Section F of the Application Instructions, an 8.5-inch x 11-inch color copy of a USGS Topographic Map and an aerial map should be included with the application. The maps need to clearly identify the project area, including the closest surface waters, general location of the gravity sewer, pumpstations, and force mains, and the downstream connection points for the receiving sewer. Please provide the required maps according to the application requirements. 15A NCAC 02T .0107 Staff Review and Permit Preparation states: "(2) (e) If an application is accepted and later found to be incomplete, the applicant shall be advised how the application or accompanying supporting information may be modified to make it complete.
