3a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:
The area consists of residential properties mostly with grass lawns, and a mixture of bulkheads and riprap. The existing conditions at
the project site is a residential home with a grass lawn and a mostly vinal bulkhead with some riprap in front. A partially repaired dock
that was damaged in 2018. A short section of the bulkhead (at the place of the proposed boat ramp) is close to failing.

4. Proposed Activity
Provide a detailed description of the proposed activity including its purpose and include the type of equipment to be used:
Re -decking of an existing 180' dock, with a few piling replacements and a couple of small additional sections. Installation of 2 boat
lifts, 1 small for a creek fishing boat and 1 larger for a sailboat. Installation of a dor-mor mooring ball. Installation of additional riprap to
shore up and extend the life of the existing bulkhead. Installation of a boat ramp.
Equipment to be used include an excavator, tractor (front end loader), and a pump for jetting in pilings.
The purpose of these projects are to allow easier access to the water for dragging a kayak into the water and going swimming. As
well as allow me to support my two boats. All uses are personal to my family at my residence.

The proposed project has been minimized to include a boat ramp that is as small as practicable with a grass lawn access up to the
ramp.
1b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques.
The proposed project's boat ramp will be constructed with the existing bulkhead in place to be used as a dam and will be upland to
the normal HWL of the river.
