The Division of Water Quality Central Office received a Stormwater Management Permit Application for
the subject project on July 2, 2010. A preliminary review of that information has determined that the
application is not complete. The following information is needed to continue the stormwater review:
1. ` Please provide the deed restriction form for high density projects with outparcels. This form is
available from the website: http://portal.ncdenr.org/weblwq/w/su/statesw/forms_docs
2. Please provide a soils report that specifically states a SHWT within the BMP footprint. Note
that the SHWT is not the same as whether or not ground water was present at the time of the
borings,
Please provide a plan sheet(s) with the drainage area (DA) delineated for the pond; and label
the site area and impervious area.
rn 4. The plans state that the I-[arris Teeter and Pharmacy sites will be permitted separately but
VP, SA0 appear to drain to the proposed ponCi: Please explain. Also, is the BUA for each of these
included in the BUA the pond is designed to treat?
✓5. The Secretary of State website state: that HCC INVESTMENTS, LLC is `On Notice' and
proposed to be dissolved. Please pror::ide more information about this. We cannot issue a
permit to a LLC that is dissolved. --
6. Provide site plan sheet(s) that show all relevant information (ie: curb & gutter, sidewalks, roads,
etc.). Also, the relevant details must be provided.

7. The drainage system around the pharmacy appear to show as existing. Are these existing or
proposed pipes?
8. Please provide detailed grading plans for the entire site, including the outparcels. The plans
should include all existing, proposed contours and spot elevations. Locations of roof drains
should also be shown.
9. Some of the duplex area, outparcels F, G, I-1, and I, and parts of the retail areas along Rockfish
Road appear to not drain to the pond. All impervious area must be captured and treated.
10. The DA and impervious area values are not shown consistently between the supplement form,
SWU-101, and the calculations. Please revise to all be consistent.
11. The supplement form shows an 8" orifice, but the plans and calculations show a 15" orifice.
Please make consistent.
12. Please provide a planting plan for the pond as required.
13. A permanent drainage easement must be provided for the pond.
14. Please fill-in the items left blank in the elevation data on the supplement form.-
15. The post -development 1-yr, 24-hr discharge rate shown on the supplement is not consistent with
the calculations.
16. The clean.out depth reading for the forebay on% he O&M form appears to be incorrect.
IT. -Please provide supporting calculations for the' 1" design storm and the draw down time for the
1" storm. The 1" design storm must draw down in 2 to 5 days. The head -used is also incorrect.
Please use H/3 as shown in the BMP Manual.
18. Please provide a table of elevations, areas, incremental volumes, and cumulative volumes for
the forebay, the main pool, and the total pond. It should include areas and volumes at key
elevations such as PPE, the 1" storm elevation, and TPE.
19. The weir from the forebay to the main pool is 3' above PPE. Typically the elevation of the weir
should be approximately +/- 0.5' the PPE. Also, provide information to show that the weir will
be stable without the use of riprap or other.
20. Please provide a hardened spillway detail for the pond.
21. The invert of the pipe discharging into the forebay is below the bottom elevation of the forebay.
22. The bottom elevations of the pond are not consistent between the plans and cross -sections.
23. Provide calculations to show that the outlet swble from the pond will be non -erosive for the 10-
yr storm.
24. Provide riprap apron sizing calculations.

Please note that this request for additional information is in response to a preliminary review. The
requested information should be received by this Office prior to August 13, 2010, or the application
will be returned as incomplete. The return of a project will necessitate resubmittal of all required items,
including the application fee.
If you need additional time to submit the information, please mail, fax, or email your request for a time
extension to the Division at the address and fax number at the bottom of this letter. The request must
indicate the date by which you expect to submit the required information. The Division is allowed 90 days
from the receipt of a completed application to issue the permit.
The construction of any impervious surfaces, other than a construction entrance under an approved
Sedimentation Erosion Control Plan, is a violation of NCGS 143-215.1 and is subject to enforcement
action pursuant to NCGS 143-215.6A.
