Dear Ms. Parker: UWO
The Division of Water Quality received your Stormwater Management Permit Application for the
subject project on June 3 ,2010. After a preliminary review the submittal was found to be
substantially incomplete. - Per DWQ requirements and policies, this Office is hereby returning the
subject application package as incomplete.
The application is incomplete based on the following items:
1. Please submit the $505 application fee payable to NCDENR.
2. All appropriate application forms must be submitted. This includes SWU=101, Supplement
Forms for each BMP, O&M Agreements, Deed Restrictions, etc. Original signatures must be
provided by a Member/Manager of the owner (HCC Investments, LLC). Please see the website
for the latest form -and required information:

3. Please make sure all items in the required items checklists are included. These checklists are
found on the main application (SWU-101) and second tab of the supplement forms.
4. A narrative description of the stormwater management for the site must be provided.
5. Please be sure to design of the project meets all the minimum requirements of the BMP Manual
and the rules.

6. Please submit 2 sets of plans and calculations that are sealed, signed, and dated.

7. Any wetlands or streams on the property must be delineated, or include a note on the plans
that none exist on the site.
8. Please delineate the drainage area on the plans for each BMP provided.
Until a State Stormwater Permit is issued for this project, the construction of any impervious surfaces,
other than a construction entrance under an approved Sedimentation Erosion Control Plan, is a
violation of NCGS 143-215.1 and is subject to enforcement action pursuant to NCGS 143-215.6A.
