Dear Mr. Tweed:
In accordance with your request received June 22, 1999, the Division is forwarding the subject permit
modification. This modification documents the change in ownership at the subject facility. Also, monitoring for
instream conductivity, Total Nitrogen and Total Phosphorus has been deleted. All other terms and conditions in
the original permit remain unchanged and in full effect. This permit modification is issued under the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North
Carolina and the U. S. Environmental Protection Agency dated December 6. 1983.
If any parts, measurement frequencies or sampling requirements contained in this permit modification
are unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30)
days following receipt of this letter. This request must be a written petition conforming to Chapter 150B of the
North Carolina General Statutes, fled with the Office of Administrative Hearings, Post Office Drawer 27447,
Raleigh, North Carolina 27611-7447. Unless such demand is made, this decision shall be final and binding.
This permit does not affect the legal requirement to obtain other permits which may be required by the
Division of Water Quality, the Division of Land Resources, Coastal Area Management Act, or any other Federal or
Local government permit that may be required. If you have any questions concerning this permit, please contact

PERMIT
TO DISCHARGE WASTEWATER UNDER THE
NATIONAL POLLUTANT DISCHARGE ELIMINATION SYSTEM
In compliance with the provision of North Carolina General Statute 143-215.1, other lawful
standards and regulations promulgated and adopted by the North Carolina Environmental
Management Commission, and the Federal Water Pollution Control Act, as amended,
Heater Utilities, Inc.
is hereby authorized to discharge wastewater from a facility located at the
Killian Crossroads
NCSR 1849
west of Terrell
Catawba County
to receiving waters designated as Lake Norman (Reed Creek Arm) in the Catawba
River Basin
in accordance with effluent limitations, monitoring requirements, and other
conditions set forth in Parts I, II, III and IV hereof.
This permit shall become effective August 11, 1999.
This permit and authorization to discharge shall expire at midnight on June 30, 2000.

SUPPLEMENT TO PERMIT COVER SHEET
Heater Utilities, Inc., is hereby authorized to:
1. Continue to operate an existing 0.05 MGD wastewater treatment system which
includes the following components:
➢ Influent surge tank
➢ Bar screen
➢ Dual aeration basins with diffused air
➢ Dual clarifiers
➢ Tertiary filters
➢ Chlorine disinfection
➢ Diffused air post -aeration
➢ Dechlorination
➢ Cascade post -aeration
➢ Aerated sludge holding tank
➢ Standby power generator
This wastewater treatment system is located at the Killian Crossroads WWTP on
NCSR 1849 west of Terrell in Catawba County.
2. After receiving an Authorization to Construct from the Division, construct the
necessary components to upgrade the wastewater treatment system to 0.075
MGD.
3. Discharge from said treatment works at the location specified on the attached
map into Lake Norman (Reed Creek Arm), classified WS-IV CA waters in the
Catawba River Basin.

submitted by Mid South. Heater will use the same plan. Most of the waste
sludge will be removed by contract septage haulers and disposed of in various
municipal systems as permitted by the septage hauler and municipality.

SLUDGE {MANAGEMENT PLAN
for
MID SOUTH WATER SYSTEMS, INC.
No sludge will be freafed on any wasfewafer, freafinenf plant sife operated by Mid Soufh Wafer Systems, Jnc.
Where practical, sludge removed from a laid South f acitify will be fransporfed via a contract hauler fo another Mid South
3acilify for fhe purpose of "seeding" a new or under loaded ptanf.
Unusable (or "dead") sludge will be removed by a confracf hauler and properly disposed of in accordance with 11C f S 143-
215.1. Confraef haulers used by laid South will be requ:ped iv pe sort the quanfify of sludge fransporfed and idenfi f y fhe
location of fhe proposed disposal site if fhe sludge is not Taken fo an existing plant operafed by Mid Soufh. Mid Soufh has
not entered info any agreement fo accept sludge info ifs f acitifies from plants not owned by fhem.
Mid Soufh Wafer Systems, Jnc, will keep records on fhe quanfify of sludge removed from each f acit fy, fhe name of fhe
confraef hauler, and fhe desfinafion of fhe sludge (whether used in another plant or disposed of). The inf ormafion will be
kepf on file and wilt be made available fo any regulatory agency having jurisdiction over sludge freafinenf or disposal.
