I, Michael L. Slusher, as a duly registered Professional Engineer in the State of North Carolina, having
been authorized to periodically observe the construction of the modifications and improvements to the Utley Creek
Water Reclamation Facility, located on Irving Parkway in Wake County for the Town of Holly Springs, hereby state
that, to the best of my abilities, due care and diligence was used in the observation of the following components
construction:
Completed and ready for service as of July 19. 2010:
1. a new influent pump station consisting of an influent Parshall Flume, two (2) 10 MGD mechanical bar
screens, two (2) 10 MGD vortex grit removal units, and six (6) 4,167 gpm, 70 Hp influent pumps, (completed
August 2009)
2. a 4.8 MGD dual train system consisting of a five stage BNR process and each train consisting of a 0.2 MG
anaerobic basin, a 0.36 MG anoxic basin, and a 2.3 MG oxidation ditch with two 150 HP mixers;
o (train AB-2 was completed August 2009)
o (train AB-3 was completed March 31, 2010)
3. modification to the existing 1.2 MGD oxidation ditch by adding a new 0.1 MG anaerobic tank; (train AB-1)
4. dual 0.2 MGD post anoxic basins and dual 0.1 MG post re -aeration basins for blending with the effluent from
all three oxidation ditch trains; (completed August 2009)
5, three 90-foot diameter, 16-foot side -water depth, final clarifiers; (completed August 2009)
6. four (4) 2.0 MGD effluent disk filters; (completed August 2009)
7. a UV disinfection system consisting of two banks in series with each rated at 15 MGD; (completed August
2009)
8. chemical storage tanks,
9. a biosolids processing system with a capacity to treat sludge from a 6.0 MGD BNR system consisting of a
0.89 MG digester (interchange tank) with decanter, screening building and equipment, and existing clarifier
converted to a 0.4 MG aerated sludge holding tank;
10.an approximately 1600 sq. ft, lab building expansion;
11.a new 1000 KW diesel generator with automatic transfer switch; (completed August 2009)
12.all associated site work, electrical work, appurtenances, and piping,
pursuant to the application received on December 18, 2006, and in conformity with the project plan, specifications,
and other supporting data subsequently filed and approved by the Department of Environment and Natural
Resources.
I certify that the construction of the above referenced project was observed to be built within substantial
compliance and intent of the approved plans and specifications.
We understand that the new limits associated with the modifications to expand to 6.0 MGD now apply with a
condition that this facility is only permitted to operate and discharge only 2.4 MGD unless modified by the future
permits and Special Orders of Consent.

The Town of Holly Springs started the year permitted to operate a 1.50 MGD water reclamation facility.
Early in Fiscal Year 2009/2010 the first phase of the facility upgrade and expansion project was
completed and the Town is now permitted to operate a 2.40 MGD water reclamation facility. The facility
is designed with a treatment capacity of 6.0 MGD and is now manned 24 hours per day. The treatment
process utilizes the activated sludge process that also performs biological nutrient removal of phosphorus
and nitrogen. The Holly Springs Water Reclamation Facility (WRF) discharges into Utley Creek which is
in the Cape Fear River Basin. The WRF discharged 440.539 MG in this calendar year, which equals an
average daily flow of approximately 1.207 MG of treated wastewater effluent.
During the calendar year July 1, 2009 to June 30, 2010 the facility exceeded one of the parameters of its
permitted regulatory standards and had one occurrence of a spill composed of return activated sludge
(RAS). The parameter that was exceeded was Total Phosphorus which is permitted on an annual mass
loading basis of 3653 pounds. The total discharged was 4384 pounds of phosphorus for the year.
In April there was a spill event of 12,000 gallons of RAS. This incident resulted in the issuance of a
Notice of Violation from the North Carolina Department of Environment and Natural Resources: Division
of Water Quality. The adjacent waterways were isolated and cleaned with assistance from the Public
Works Department under the guidance of the North Carolina Department of Environment and Natural
Resources: Division of Water Quality. The stream suffered no permenant environmental damage and
remains a healthy natural environmental habitat.

Dear Ms. Sudano:
A letter of request for Authorization to Construct was received on December 18, 2006, by
the Division, and final plans and specifications for the subject project have been reviewed and
found to be satisfactory. Authorization is hereby granted for the construction of modifications to
the existing 1.5 MGD Utley Creek Water Reclamation Facility, with discharge of treated
wastewater up to 2.4 MGD into the Utley Creek River in the Cape Fear River Basin.
This Authorization to Construct results in an increase in design to 6.0 MGD, and is
awarded with the condition that this facility is only permitted to operate and discharge up to 2.4
MGD unless modified by future permits or Special Orders of Consent. Further, in accordance
with the FNSI dated February 16, 2007, the treated effluent must be removed from Utley Creek
by the date established in the Certificate Authorizing the Towns of Cary, Apex, and Morrisville
and Wake County to Increase Their Transfer of Water from the Haw River basin to the Neuse
River basin under the Provisions of G.S. 143-215,221. This authorization is awarded for the
following modifications:
A new influent pump station consisting of an influent Parshall Flume, two (2) 10 MGD
mechanical bar screens, two (2) 10 MGD vortex grit removal units, and six (6) 4,167 gpm, 70 Hp
influent pumps; a 4.8 MGD dual train system consisting of a five stage BNR process and each
train consisting of a 0.2 MG anaerobic basin, a 0.36 MG anoxic basin, and a 2.3 MG oxidation
ditch with two 125 HP mixers and tine bubble aeration; modification to the existim- 1.2 MGD
oxidation ditch by adding a new 0.1 MG anaerobic tank; dual 0.2 MG post anoxic basins

effluent disk filters; a UV disinfection system consisting of two banks in series with each rated at
15 .MGD; chemical storage tanks, and a biosolids processing system with a capacity to treat
sludge from a 6.0 MGD BNR system consisting of a 0.89 MG digester (interchange tank) with
decanter, screening building and equipment, and existing clarifier converted to a 0.4 MG aerated
sludge holding tank; an approximately 1600 sq. ft. lab building expansion; a new 1000 KW
diesel generator with automatic transfer switch; all associated site work, electrical work,
appurtenances, and piping, pursuant to the application received on December 18, 2006, and in
conformity with the project plan, specifications, and other supporting data subsequently filed and
approved by the Department of Environment and Natural Resources.
This Authorization to Construct is issued in accordance with Part II1, Paragraph A of
NPDES Permit No. NCO063096 and shall be subject to revocation unless the wastewater
treatment facilities are constructed in accordance with the conditions and limitations specified in
Permit No. NC0063096.
The sludge generated from these treatment facilities must be disposed of in accordance
with G.S. 143-215.1 and in a manner approved by the North Carolina Division of Water Quality.
In the event that the facilities fail to perform satisfactorily, including the creation of
nuisance conditions, the Permittee shall take immediate corrective action, including those as may
be required by this Division, such as the construction of additional or replacement wastewater
treatment or disposal facilities.
The Raleigh Regional Office, telephone number (919) 791-4200, shall be notified at least
forty-eight (48) hours in advance of operation of the installed facilities so that an on site
inspection can be made. Such notification to the regional supervisor shall be made during the
normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State
Holidays.
Upon completion of construction and prior to operation of this permitted facility, a
certification must be received from a professional engineer certifying that the permitted facility
has been installed in accordance with the NPDES Permit, this Authorization to Construct and the
approved plans and specifications. Mail the Certification to Cecil G. Madden, Jr., P.E.,

1633.
Upon classification of the wastewater treatment facilities by the Water Pollution Control
System Operators Certification Commission (WPCSOCC), the Permittee shall designate and
employ a certified operator to be in responsible charge (ORC) and one or more certified
operator(s) to be back-up ORC(s) of the facilities in accordance with 15A NCAC 8G .0201. The
ORC shall visit the facilities in accordance with 15A NCAC 8G .0204 or as specified in this
permit and shall comply with all other conditions specified in these rules.

A copy of the approved plans and specitications shall be maintained on file by the
Permittee for the life of the facility.
During the construction of the proposed additions/modifications, the permittee shall
continue to properly maintain and operate the existing wastewater treatment facilities at all times,
and in such a manner, as necessary to comply with the effluent limits specified in the NPDES
Permit.
You are reminded that it is mandatory for the project to be constructed in accordance
with the North Carolina Sedimentation Pollution Control Act, and when applicable, the North
Carolina Dam Safety Act. In addition, the specifications must clearly state what the contractor's
responsibilities shall be in complying with these Acts.
Prior to entering into any contract(s) for construction, the recipient must have obtained all
applicable permits from the State.
Failure to abide by the requirements contained in this Authorization to Construct may
subject the Permittee to an enforcement action by the Division of Water Quality in accordance
with North Carolina General Statute 143-215.6A to 143-215.6C.
The issuance of this Authorization to Construct does not preclude the Permittee from
complying with any and all statutes, rules, regulations, or ordinances which may be imposed by
other government agencies (local, state, and federal) which have jurisdiction.
