Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:* Existing conditions include SR 2106 (Macedonia Church Road), agricultural land, residential property, undeveloped forest, and the disturbed/maintained right-of-way. This project was formerly known as T.I.P. #B-5515 and 17BP.10.R.151 4b. Explain the purpose of the proposed project: * The purpose of the project is to replace the structurally deficient SR 2106 bridge over Little Richardson Creek with a new bridge. 4i. Describe the overall project in detail, including indirect impacts and the type of equipment to be used: * Erosion and sedimentation control measures will be installed, including the installation of impervious dikes around the existing bridge abutments in order to work in dry conditions. Bridge demolition will include the removal of the bridge deck and the removal of the top portion of the bridge abutments, leaving the Tooter and approximately three feet of the concrete abutment to match the excavation elevation. Temporary construction and demolition -related impacts to 85 linear feet of Little Richardson Creek may result. Bridge replacement construction will utilize heavy equipment operating from the upland roadway approaches. No deck drains are proposed on the bridge and runoff from the bridge will be routed to inlets located on both sides of the road to the northeast of the bridge and discharged to the stream floodplain both upstream and downstream of the bridge. The proposed bridge will be built on the same alignment with a temporary off -site detour that includes Pageland Highway, East Franklin Street, Wolf Pond Road, and Stack Road. The existing 24.5' single -span bridge over Little Richardson Creek on SR 2106 (Macedonia Church Rd) is being replaced with a 60' single -span bridge. The bridge will have 1.5:1 sloping abutments. The roadway typical section through the bridge will be normal crowned at 2 % with 10' travel lanes and 3'-11" shoulders, 27"-10" clear width and 30'-0" OTO width. The existing bridge structure will be removed. The existing abutments will be cut off at elevation 540. Overall drainage patterns will be maintained. Existing roadside ditches will be replaced by proposed roadside ditches which will be appropriately lined to prevent erosion. On the end side of the bridge, two traffic bearing grated drop inlets with double frame and grates (2GI) were placed within the shoulder berm gutter to pick up runoff from the bridge. Class -II rip -rap abutment protection will be placed on both sides of the stream crossing. Deck drains are not required. Rip -rap dissipator pads are proposed at the ends of the ditches where the ditches outfall towards the stream. Impervious dikes will be installed around the existing abutments to prevent the sediment from washing into the stream during the removal of existing structure. 5. Jurisdictional Determinations 5a. The bridge span length was extended to avoid impacts. Riprap protection of the bridge abutments is being placed outside of the jurisdictional stream boundary to avoid impacts to the streambed. 1b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques: NCDOT Best Management Practices for Construction and Maintenance Activities will be employed. Demolition and construction methods from the upland roadway approaches will be used to minimize potential temporary demolition related impacts and avoid construction related impacts. 2.
