10. Comments: Overall the site has sufficient permanent ground cover to restrain erosion,however there are
some bare areas that need to be stabilized in-between the Shade Valley Road roundabout and
Monroe Road. No additional self-monitoring or self-inspections are required.Project is in
compliance and released under the SPCA.Termination of coverage under the NPDES General
Stormwater Permit for Construction Activities(NCGO1)must be requested by the permittee
via a Notice of Termination(NOT)unless permit approval date was prior to April 2019.
The NOT is available at:https://edocs.deq.nc.gov/ForTns/NCGOI-Termination.Please note
that this correspondence must be uploaded as part of completing the NOT process.

Good morning,
The 2023 annual fee was never paid for Shade Valley or Providence Road Sidewalk, and the permits were
permanently expired. They were prevented from renewing under the new NCG010000 General Permit effective
April 1. That's why you weren't able to submit an NOT request.The invoice attempts and Notice of Deficiency
letters from last year, as well as emails about the permanent expiration are attached.
The permit file for the Providence Road Sidewalk project is: NCC215620
The permit file for the Shade Valley project is: NCC215773
(These permits above are no longer active.)
The permit file for the Oakhurst/Amity Gardens project is: NCC211612 (Still active)

Attached are the closeout inspection reports foryour sites peryou request.
Anything approved prior to April of 2019 does not require a e-NOT for NCG01 coverage.
Regarding the error message you received I will leave it up to Raleigh SW contacts to determine what actions are
necessary.

Good afternoon Jeffrey,
I'm trying to get NOTs for three projects: Shade Valley Road Realignment& Roundabout, Oakhurst/Amity Gardens Multi-
Use Path, and Providence Road Sidewalk. I have the final inspection reports for the first two, but I need a copy of the
one for Providence Road Sidewalk.

Also, I tried using the DEQ's website to obtain NOT's for the first two, but on the NOT request form, I keep getting this
message : DO NOT PROCEED with this Notice of Termination.This Certificate of Coverage is no longer active.
What does this mean? Did I miss a step? I'd like to get this resolved before the City Engineer receives another e-mail
notification.
The Certificate of Coverage Number for the Shade Valley project is: NCC215773.
The Certificate of Coverage Number for the Oakhurst/Amity Gardens project is: NCC211612.
