This inspection was conducted to verify that the facility is operating in compliance with the conditions and limitations specified in NPDES WW Permit No. NCG550801. A summary of the findings and comments noted during the inspection are provided in the enclosed copy of the inspection report. The Compliance Evaluation inspection was conducted by Division of Water Resources staff from the Asheville Regional Office. The Division of Water Resources may pursue enforcement action for this and any additional violations of State law. NCG550801 I11 12I 24/03/25 117 18 i c i Section D: Summary of Finding/Comments (Attach additional sheets of narrative and checklists as necessary) Rachel Rose and Natalie Nish, with the Asheville Regional Office (ARO), conducted a Compliance Evaluation Inspection of a single-family residential (SFR) wastewater disposal system that services Blessed Hope Church at 2312 Blowing Rock Blvd, Lenoir, NC 28645. This inspection was conducted to determine if the system is being operated and maintained in compliance with General NPDES Permit No. NCG550000. Pastor Charlie Woodring, owner, was contacted to assist with the inspection details. This system was originally permitted on May 4, 1996 which included a septic tank, subsurface sand filter, tablet chlorinator, and a cascade aeration. A copy of the updated General Permit NCG55 requirements will be provided with this report. Pastor Charlie Woodring has been a pastor at Blessed Hope Advent Christian Church since 2016. For future reference, the mailing address needs to be updated with the current pastor's address if that were to change in the future since the church has no mail deliverable address. The following items were noted during the inspection: 1. Flow Measurement - Effluent: If the system were to ever discharge, an annual flow measurement would be required (see Section C. [1] of the General Permit NCG55). 2. Effluent Samples: At the time of the inspection, the system was not discharging. If the system were to ever discharge, effluent samples would be required. With the receiving stream (Blair Fork [11-39-3-11) being a Class C stream, the effluent sampling requirements can be found in Section C. (1.) of the General Permit NCG55. 3. Effluent Pipe: Be sure to clear the vegetation that was covering the effluent pipe at the time of the inspection to allow for proper inspection of the outfall. The following deficiency was noted during the inspection: 1. Septic Tank: There was no knowledge of any inspections or pumping services to the septic tank from Charlie's knowledge. The septic tank needs to be located, checked annually, and pumped at least once every five years (see Section D. [3] of the General Permit NCG55).
