Inspection Summary:
A routine compliance inspection of the S & J Villari Livestock WWTF was conducted on March 12, 2024. The purpose of the
evaluation was to verify that the facility is operating in compliance with the conditions and limitations specified in Permit
WQ0016376. Gerald Matthews, ORC, was present during the inspection.
This facility is a facultative lagoon, spray irrigation system for industrial waste. In January of this year, the average flow was
66,575 GPD of the 100,260 GPD permitted flow. Measuring of the solids in the synthetically -lined lagoon will be done in the
near future. The Permittee is considering sending the solids to Carbon Cycle, located across the street from the facility,
when they complete construction. There have been some odor complaints from neighboring residents. The facility has a
spray odor system along the edge of the spray fields to help with the odor. There appears to be a need for more signs/gates
to restrict public access to the irrigation fields, especially Fields 14E and 14W, along Blackmore Road.
A review of records showed that all of the 2022 and 2023 GW-59s were not submitted as required. The ORC electronically
sent these copies to the Wilmington Regional Office (WIRO) after the inspection. A review of the missing GW-59s show
some intermittent exceedances of Nitrate in MW-2, MW-8 and MW-9 and Ammonia exceedances in MW-7. The pH level
tends to be low in all of the monitoring wells. The WIRO will continue to watch the trends of these wells. MW-5 has not been
sampled since July 2022. This issue needs to be corrected as soon as possible. Please submit the quarterly GW-59s on
time going forward.
The Annual Soil Analysis Report was not available for review. The ORC stated that lime will be applied when field conditions
are adaquate. Results of these reports are required to be maintained at the facility for five years and be made available upon
request.
The facility is non -compliant with Non -Discharge Permit W00016376. A Notice of Deficiency will be issued for the issues
discussed above. Please review the comment sections of the inspection report for more details.

in good operating condition. The solids are separated before entering the lagoon and are

Comment: Effluent is pumped from the 1.88 MG stainless steel storage tank throuqh the 21,000 gallon
chlorine contact tank before irrigation. Liquid chlorine is used. The visual and audible alarms
for the contact chamber tank were operational when tested. The grab samples for TRC is
collected at a spigot off of the contact chamber. The TRC is analyzed by Envirochem Lab staff
weekly on site.

Comment: Operational and maintenance records consists of dailv check sheets and Preventative
Maintenance Work Orders that are dated and signed when completed. The field parameters,
TRC and pH, are analyzed by Envirochem Lab staff weekly. Envirochem calibrations, bench
sheets and Chain of Custodies were reviewed and appear to be complete. GW-59 reports for
2022 and 2023 were not submitted, but the monitoring wells were sampled and tested by
Envirochem. The ORC forwarded the reports electronically to WIRO after the inspection for
review and is in the process of uploading them to Laserfiche. After review, it was found that
MW-5 had not been sampled since July 2022. An annual soil analysis was not availble for
review.

Comment: The Center Pivot, for Fields 14E and 14W, was observed to have some clopped or missing
spray heads while in operation during the inspection. The ORC had the pivot serviced and
cleaned as part of a PM Work Order, and sent verification that it was completed the following
week. The crop on Fields 1-13 and 15-17 need to be cut and baled. ORC indicated that they
are waiting on acceptable field conditions to allow maintenance of the irrigation areas. Please
remember that, according to Condition 11.4 of the permit, the Permittee shall maintain the
vegetative cover on the irrigation sites such that crop health is optimal, allows even effluent
distribution and allows inspection of the irrigation system. The permit allows for double
cropping to have a healthy crop for irrigation year round, which increases the PAN limit from
300 to 350 Ibs/acre.
