A site visit was not conducted as part of this review. None of the C.O.C's indicated a method of preservation. It is essential that the preservation method is noted on the C.O.C. for any sample submitted for analysis. The laboratory intake sheets indicate that samples were received at acceptable temperatures. However, a note needs to be made on the C.O.C. indicating method of preservation. If the C.O.C. does not provide a section to indicate preservation method, a hand written note indicating the preservation method is acceptable.
